摘要
目的 探讨基于鼻腔通气功能、炎性因子水平变化及短期复发率评价慢性鼻-鼻窦炎鼻息肉(CRSwNP)患者围手术期使用布地奈德鼻喷雾剂的影响效果。方法 选取2022年1月至2023年6月我院耳鼻喉科收治的CRSwNP患者68例,随机信封法分为对照组与联合组,各34例。对照组给予单独鼻内镜手术治疗,联合组在对照组基础上加用布地奈德鼻喷雾剂治疗,比较两组临床疗效、鼻腔通气功能指标[鼻腔最小横截面积(NMCA)、鼻腔容积(NCV)、鼻腔最小横截面到前鼻孔的距离(DCAN)、鼻气道总阻力(NAR)]、炎性因子[C反应蛋白(CRP)、嗜酸性粒细胞阳离子蛋白(ECP)、肿瘤坏死因子-α(TNF-α)]水平及短期复发率。结果 术后30 d,联合组临床治疗有效率高于对照组(P <0.05)。联合组NMCA、NCV、DCAN、NAR值均优于对照组(P <0.05)。联合组CRP、ECP、TNF-a水平均低于对照组(P <0.05)。联合组短期复发率低于对照组(P <0.05)。结论 对CRSwNP患者围手术期使用布地奈德鼻喷雾剂可显著提高其临床治疗效果,改善鼻通气功能,降低炎性因子水平及复发率。
Objective To evaluate the effect of perioperative budesonide nasal spray on patients with chronic rhinosinusitis and nasal polyps(CRSwNP) based on nasal ventilation function,changes in inflammatory factors and short-term recurrence rate.Methods A total of 68 patients with CRSwNP admitted to Otolaryngology Department of our hospital from January 2022 to June 2023 were selected and divided into control group and combination group by random envelope method,with 34 cases in each group.The control group was treated with nasal endoscopic surgery alone,and the combined group was treated with budesonide nasal spray on the basis of the control group.The clinical efficacy,nasal ventilation function indexes [minimum nasal cross-sectional area(NMCA),nasal volume(NCV),distance from minimum nasal cross section to anterior nostril(DCAN),total nasal airway resistance(NAR)],inflammatory factors [C-reactive protein(CRP),eosinophilic cationic protein(ECP),tumor necrosis fact-α(T) were compared between the two groups NF-α] levels and short-term recurrence rate.Results On the 30d after operation,the effective rate of combined group was higher than that of control group(P<0.05).The values of NMCA,NCV,DCAN and NAR in combination group were better than those in control group(P<0.05).The levels of CRP,ECP and TNF-a in combination group were lower than those in control group(P<0.05).The short-term recurrence rate of combination group was lower than that of control group(P<0.05).Conclusions The perioperative use of budesonide nasal spray in patients with CRSwNP can significantly improve the clinical therapeutic effect,improve the nasal ventilation function,reduce the level of inflammatory factors and recurrence rate.
