摘要
目的分析功能性鼻内镜手术(FESS)联合布地奈德浸润治疗慢性鼻窦炎伴鼻息肉(CRSwNP)的效果及对患者嗅觉功能、鼻腔通气功能、复发的影响。方法选取本院2017年1月—2020年1月收治的106例CRSwNP,根据治疗方法的不同分为联合组54例和单一组52例,联合组采用FESS联合布地奈德浸润治疗,单一组仅采用FESS治疗。比较2组临床疗效,治疗前后嗅觉功能评分、鼻黏膜纤毛清除率、鼻黏膜纤毛清除速度、鼻腔最小横截面积(NMCA)、鼻腔容积(NCV)、鼻腔最小横截面至前鼻孔距离(DCAN)、鼻气道阻力,以及术后并发症和复发情况。结果治疗后联合组总有效率显著高于单一组(P<0.05)。2组治疗后嗅觉功能评分、DCAN、鼻气道阻力较治疗前明显下降,鼻黏膜纤毛清除率、鼻黏膜纤毛清除速度、NMCA、NCV均较治疗前明显升高,且联合组上述指标改善情况均优于单一组(P<0.05,P<0.01)。联合组并发症总发生率及复发率均显著低于单一组(P<0.05,P<0.01)。结论FESS联合布地奈德浸润治疗CRSwNP效果确切,可有效改善患者嗅觉功能,促进鼻腔通气功能恢复,降低术后并发症发生率及复发率。
Objective To analyze clinical efficacy of functional endoscopic sinus surgery(FESS)combined with Budesonide infiltration in treatment of patients with chronic sinusitis associated by nasal polyps(CRS-NP)and its effects on olfactory function,nasal ventilation function and recurrence.Methods A total of 106 patients with CRS-NP admitted between January 2017 and January 2020 were divided into combination group(n=54)and single group(n=52)according to different therapeutic methods.Combination group was treated with FESS combined with Budesonide infiltration,while single group was treated with FESS only.Clinical efficacy,olfactory function score,nasal mucosal cilia clearance rate,nasal mucosal cilia clearance speed,nasal minimum cross-sectional area(NMCA),nasal cavity volume(NCV),distance from nasal minimum cross-section to anterior nostril(DCAN)and nasal airway resistance before and after treatment,as well as postoperative conditions of complications and recurrence were compared between two groups.Results After treatment,the total effective rate in combination group was significantly higher than that in single group(P<0.05).After treatment,values of olfactory function scores,DCAN and nasal airway resistance were significantly decreased,while values of nasal mucosal cilia clearance rates,nasal mucosal cilia clearance speed,NMCA and NCV of nasal mucosa were significantly increased compared with those before treatment in two groups;the improved conditions of the above indexes in combination group were better than those in single group(P<0.05,P<0.01).The total incidence rate of complications and recurrence rate in combination group were significantly lower than those in single group(P<0.05,P<0.01).Conclusion FESS combined with Budesonide infiltration is effective in treatment of patients with CRS-NP.It may effectively improve olfactory function,promote recovery of nasal ventilation function and reduce incidence rate of complications and postoperative recurrence rate.
