加味千金苇茎汤联合高压氧对急性呼吸窘迫综合征大鼠肺肠的保护作用及机制研究

Efficacy and mechanism of modified Qianjin Weijing Decoction combined with hyperbaric oxygen therapy in protecting pulmonary and intestinal tissues in rat models of acute respiratory distress syndrome

摘要 目的基于肺肠相表里中医药经典腑脏理论,观察加味千金苇茎汤联合高压氧对急性呼吸窘迫综合征(ARDS)大鼠肺肠的保护作用及机制。方法选择Wistar大鼠40只,32只构建ARDS模型后随机分为4组,每组8只,即模型对照组、中药组、高压氧组及高压氧联合中药组,模型对照组不进行干预,高压氧组采用高压氧干预,高压氧联合中药组采用高压氧联合中药进行干预,剩余8只为正常对照组。观察各组的存活率并监测各组血氧饱和度和血气分析值,检测大鼠血浆D-乳酸、内毒素及二胺氧化酶(DAO)水平,同时光镜下观察肺、结肠组织的病理变化,采用酶联免疫吸附测定(ELISA)检测肺灌洗液中的肿瘤坏死因子-α(TNF-α)、白介素-1β(IL-1β)、白介素-6(IL-6)、白介素-10(IL-10)表达水平的变化,进行肺组织病理评分分析和计算肺湿/干重比(W/D),并测定肺组织脂质过氧化水平。结果高压氧组和高压氧联合中药组存活率为100%,模型对照组存活率为50%,中药组存活率为75%。正常对照组大鼠肺、结肠组织结构完整,模型对照组大鼠肺组织存在大量炎症细胞,高压氧联合中药组大鼠肺肠组织炎症水平显著低于中药组和高压氧组,其D-乳酸、内毒素、DAO水平、肺W/D、肺组织病理评分明显低于中药组和高压氧组,差异有统计学意义(P<0.05)。高压氧联合中药组动脉血氧分压/吸入气氧浓度(PaO2/FiO2)、PaO2显著高于模型对照组和高压氧组,差异有统计学意义(P<0.05)。高压氧联合中药组大鼠肺组织髓过氧化物酶(MPO)、丙二醛(MDA)、谷胱甘肽(GSH)水平明显低于模型对照组和高压氧组,而过氧化氢酶(CAT)和超氧化物歧化酶(SOD)活性显著高于模型对照组和高压氧组,高压氧联合中药组大鼠肺灌洗液的IL-1β、IL-6和TNF-α水平明显低于模型对照组和高压氧组,差异有统计学意义(P<0.05)。模型对照组、中药组、高压氧组及高压氧联合中药 Objective To investigate the protective effects and underlying mechanisms of modified Qianjin Weijing Decoction combined with hyperbaric oxygen therapy on pulmonary and intestinal tissues in rat models of acute respiratory distress syndrome(ARDS)based on the traditional Chinese medicine(TCM)classic visceral theory of the lung-intestine correlation.Methods Forty Wistar rats were involved in this study,with thirty-two being utilized to establish the ARDS model.These rat models were subsequently randomized into four groups of eight:a model control group,a decoction group,a hyperbaric oxygen group,and a hyperbaric oxygen combined decoction group.The model control group underwent no intervention,whereas the hyperbaric oxygen group received hyperbaric oxygen therapy,and the hyperbaric oxygen combined decoction group was treated with both hyperbaric oxygen and TCM.The rest eight rats were used as blind controls.Survival rates,blood oxygen saturation,and blood gas analysis values were monitored across all groups.Plasma D-lactic acid,endotoxin,diamine oxidase(DAO)levels were detected.Microscopic examination was employed to assess pathological changes in lung and colon tissues.Enzyme-linked immunosorbent assay(ELISA)was used to quantify the expression levels of tumor necrosis factor-alpha(TNF-α),interleukin-1 beta(IL-1β),interleukin-6(IL-6),and interleukin-10(IL-10)in lung lavage fluid.Additionally,lung tissue pathology scoring and lung wet/dry weight ratio(W/D)were calculated,and lipid peroxidation levels in lung tissue were measured.Results The survival rates in the hyperbaric oxygen group and hyperbaric oxygen combined decoction group were 100%,in contrast to 50%in the model control group and 75%in the decoction group.In the normal control group,the structural integrity of lung and colon tissues was preserved.In contrast,the model control group exhibited significant inflammatory cell infiltration in lung tissues.Inflammation levels in the pulmonary and intestinal tissues of the hyperbaric oxygen combined decoction gro
