摘要
目的:探讨高龄2型糖尿病(T2DM)患者身体质量指数(BMI)与骨质疏松(OP)及血清25-羟基维生素D[25-(OH)D]水平的相关性。方法:选取2019年1月—2020年12月某三甲医院内分泌科收治的200例高龄T2DM患者作为研究对象,按照《原发性骨质疏松症诊疗指南(2022)》中关于骨质疏松等诊断标准分为OP组和非OP组,各100例。测量两组患者的BMI、血清25-(OH)D水平、骨密度(BMD)、骨转换生化标志物(β型Ⅰ型胶原交联羧基端肽β-CTX、Ⅰ型前胶原氨基端原肽PINP)等指标,并进行统计学分析。结果:OP组BMI、血清25-(OH)D水平、BMD、T值低于非OP组,β-CTX、PINP水平高于非OP组,异有统计学意义(P<0.05)。BMI、血清25-(OH)D水平与BMD呈正相关(r=0.421、0.386,P<0.001),与β-CTX、PINP水平呈负相关(r=-0.352、-0.328,P<0.001)。BMI、血清25-(OH)D水平是高龄T2DM患者OP发生的独立影响因素(P<0.05)。结论:高龄T2DM患者BMI下降、血清25-(OH)D水平降低是导致OP发生的独立危险因素,BMI、血清25-(OH)D水平与BMD呈正相关,而与骨转换标志物呈负相关。
Objective:To investigate the correlation between body mass index(BMI),osteoporosis(OP)and serum 25-hydroxyvitamin D[25-(OH)D]levels in elderly patients with type 2 diabetes mellitus(T2DM).Methods:A total of 200 elderly T2DM patients admitted to the endocrinology department of a top-three hospital from January 2019 to December 2020 were selected as the study objects.According to the diagnostic criteria of osteoporosis in the Guidelines for Primary Osteoporosis Diagnosis and Treatment(2022),the patients were divided into OP group and non-OP group,with 100 cases in each group.BMI,serum 25-(OH)D level,bone mineral density(BMD)and biochemical markers of bone turnover(β-CTX,β-CTX and PINP)of type I collagen were measured in 2 groups,and statistical analysis was performed.Results:BMI,serum 25-(OH)D level,BMD and T value in OP group were lower than those in non-OP group,andβ-CTX and PINP levels were higher than those in non-OP group,with statistical significance(P<0.05).BMI and serum 25-(OH)D levels were positively correlated with BMD(r=0.421,0.386,P<0.001).They were negative correlation withβ-CTX and PINP levels(r=-0.352,-0.328,P<0.001).BMI and serum 25-(OH)D level were independent risk factors for OP in elderly patients with T2DM(P<0.05).Conclusion:Decreased BMI and decreased serum 25-(OH)D level in elderly T2DM patients are independent risk factors for OP.BMI and serum 25-(OH)D level are positively correlated with BMD,but negatively correlated with bone turnover markers.
