老年2型糖尿病患者血清CYR61、caveolin-1水平与骨质疏松症的关系 被引量:10

The correlations between serum CYR61 and caveolin-1 levels and osteoporosis in elderly patients with type 2 diabetes mellitus

摘要 目的探讨血清高半胱氨酸蛋白61(CYR61)、小凹蛋白-1(caveolin-1)水平与老年2型糖尿病(T2DM)患者合并骨质疏松症的关系。方法选取2020年3月至2022年8月就诊的208例T2DM患者作为研究对象。记录患者一般资料,根据骨密度检测结果分为骨量正常组(骨密度T值>1)60例、骨量减少组(-2.5<骨密度T值≤-1)62例、骨质疏松组(骨密度T值≤-2.5)86例。采用酶联免疫吸附法检测血清CYR61、caveolin-1水平,采用全自动电化学发光仪检测骨代谢标志物Ⅰ型前胶原N端前肽(PINP)、骨钙素(BGP)、25羟基维生素D_(3)[25(OH)D_(3)]、Ⅰ型胶原羧基端肽β特殊序列(β-CTX)的水平;采用Pearson法分析T2DM合并骨质疏松症患者血清CYR61、caveolin-1水平与各指标的相关性;采用logistic多因素回归分析T2DM患者合并骨质疏松症的影响因素。结果骨质疏松组糖尿病病程、β-CTX显著高于骨量减少组和骨量正常组,血清CYR61、caveolin-1、PINP、BGP、25(OH)D_(3)水平显著低于骨量减少组和骨量正常组(P<0.05)。骨量减少组糖尿病病程、β-CTX显著高于骨量正常组,血清CYR61、caveolin-1、PINP、BGP、25(OH)D_(3)水平显著低于骨量正常组(P<0.05)。T2DM合并骨质疏松症患者CYR61与caveolin-1水平呈正相关(r=0.500,P<0.05),二者与PINP、BGP、25(OH)D_(3)呈正相关,与糖尿病病程、β-CTX呈负相关(P<0.05)。CYR61、caveolin-1是T2DM患者合并骨质疏松症的保护因素(P<0.05)。结论老年T2DM合并骨质疏松症患者血清CYR61、caveolin-1低表达,可一定程度反映患者骨代谢情况。 Objective To investigate the correlations between serum homocysteine 61(CYR61)and caveolin-1 levels and osteoporosis in elderly patients with type 2 diabetes mellitus(T2DM).Methods A total of 208 T2DM patients from March 2020 to August 2022 were enrolled.The general data of the patients were recorded.According to the bone mineral density test results,they were grouped into 60 cases of normal bone density group(T>1),62 cases of osteopenia group(-2.5<T≤-1),and 86 cases of osteoporosis group(T≤-2.5).Serum CYR61 and caveolin-1 levels were detected by enzyme-linked immunosorbent assay.And automatic electrochemiluminescence instrument was applied to assess the levels of bone metabolism markers Type I procollagen N-terminal propeptide(PINP),osteocalcin(BGP),25-hydroxyvitamin D_(3)[25(OH)D_(3)],and Type I collagen carboxy-terminal peptide beta specific sequences(β-CTX).Pearson method was applied to analyze the correlations between serum CYR61 and caveolin-1 levels and each index in T2DM patients with osteoporosis.Logistic multivariate regression analysis was used to analyze the influencing factors of osteoporosis in T2DM patients.Results In osteoporosis group,the duration of diabetes mellitus andβ-CTX were significantly higher,and the serum levels of CYR61,caveolin-1,PINP,BGP,and 25(OH)D_(3) were significantly lower than those in the osteopenia group and the normal bone mass group(P<0.05).In the osteopenia group,the duration of diabetes mellitus andβ-CTX were significantly higher,and the serum levels of CYR61,caveolin-1,PINP,BGP,and 25(OH)D_(3) were significantly lower than those in the normal bone density group(P<0.05).There were positive correlations between CYR61 and caveolin-1 levels in T2DM patients with osteoporosis(r=0.500,P<0.05);and the two were positively correlated with PINP,BGP,and 25(OH)D_(3),and were negatively correlated with the course of diabetes mellitus andβ-CTX(P<0.05).CYR61 and caveolin-1 were significant protective factors of osteoporosis in T2DM patients(P<0.05).Conclusion The serum CYR61 a
