摘要
张凤武主任医师系全国第六批名老中医药专家学术经验继承指导老师,在长达50多年的临床工作中积累了丰富的治疗肠道疾病的经验。张老认为慢性泄泻在中医上虽被分为脾胃虚弱、肾阳虚衰、肝脾不合三型,但“邪之所凑,其气必虚”,所以究其根本,诸因皆为影响脾胃运化,助生湿邪,导致湿邪下注肠道发为泄泻。故慢性泄泻的病机根本在于脾胃虚弱,治疗原则旨在健脾助运化水湿。在临床工作中,张老发现在运用健脾、祛湿、补肾、疏肝理气法治疗慢性泄泻的基础上,利用祛风药醒脾散滞,疏肝解郁、升阳除湿、升阳举陷、固肾助气化以及引经之效,灵活配伍使用祛风药,有助于健脾升清、祛风胜湿、疏肝理气、固肾助气化的功效,能有效提高慢性泄泻在临床上的治疗效果。文章浅析这一理论并举一验案以资佐证,为临床治疗提供思路与方向。
Chief physician ZHANG Fengwu is the sixth group of Chinese medicine experts academic experience inheritance guidance,in more than 50 years of clinical work has accumulated rich experience in the treatment of intestinal diseases.ZHANG believes that chronic diarrhea is divided into three types in traditional Chinese medicine:weakness of spleen and stomach,deficiency of kidney Yang(阳)and incompatibility of liver and spleen.However,where pathogenic factors accumulate,the parts of the body must be deficient in the vital Qi(气),so the fundamental cause is that the function of spleen and stomach is blocked,dampness is injected into the intestine,and finally diarrhea is formed.Therefore,the pathogenesis of chronic diarrhea is weakness of spleen and stomach,and the principle of treatment is invigorating the spleen for eliminating dampness.In clinical work,ZHANG found that on the basis of using the methods of invigorating spleen,dispelling dampness,tonifying the kidney,dispersing liver and regulating Qi to treat chronic diarrhea,TCM medicinal materials of dispersing pathogenic wind can be used to enlivening spleen and dispersing stagnation.The compatibility of TCM medicinal materials of dispersing pathogenic wind with dispersing stagnated liver Qi for relieving Qi stagnation,invigorating vital function and removing dampness,ascending up spleen Qi and Yang,tonifying kidney and Qi,channel ushering drugs can invigorate the spleen and ascending clear,expel wind-damp,disperse liver and regulating Qi,tonify kidney and Qi,and effectively improve the therapeutic effect.In this paper,this theory is analyzed and a case is attached to provide reference ideas for clinical treatment.
