摘要
目的:探讨集束化护理干预对膝关节骨性关节炎(KOA)患者全膝关节置换术后疼痛的影响。方法:选取医院收治的46例KOA患者,按随机数表法分为观察组和对照组,每组23例患者。对照组患者采取药物镇痛联合常规护理干预,观察组患者采取集束化护理。比较两组患者术前、术后24h、术后72 h、术后1周的静息及运动状态疼痛VAS评分及术前、术后1个月的膝关节功能评分(KSS)。结果:两组患者术前静息状态疼痛VAS评分、运动状态疼痛VAS评分差异无统计学意义(P0.05);而观察组患者术后24h、术后72h、术后1周的静息状态疼痛VAS评分、运动状态疼痛VAS评分低于对照组(P<0.05)。两组患者术前KSS临床评分与功能评分的差异无统计学意义(P>0.05);两组患者术后1个月的KSS临床评分与功能评分均较术前升高(P<0.05),观察组患者术后1个月的KSS临床评分与功能评分高于对照组(P<0.05)。结论:集束化护理干预可有效减轻患者术后疼痛,使患者尽早开展康复训练,从而促进膝关节功能恢复。
Objective:To observe the effect of cluster nursing intervention on pain after total knee arthroplasty in patients with knee osteoarthritis(KOA).Methods:A total of 46 KOA patients admitted to hospital were selected and divided into observation group and control group according to random number table method,with 23 cases in each group.The control group was treated with drug analgesia combined with routine nursing intervention,and the observation group was treated with cluster nursing.The resting and exercise pain VAS scores before surgery,24 h after surgery,72 h after surgery,1 week after surgery and knee function score(KSS)before surgery and 1 month after surgery were compared between two groups.Results:There was no significant difference in the score of pain VAS in resting state and exercise state before surgery(P≥0.05).The resting pain VAS score and exercise pain VAS score of the observation group were lower than those of the control group at 24 h,72 h and 1 week after surgery(P<0.05).There was no significant difference in preoperative KSS clinical score and functional score between the two groups(P>0.05).The clinical scores and functional scores of KSS in 2 groups 1 month after surgery were higher than those before surgery(P<0.05).The clinical scores and functional scores of KSS in the observation group were higher than those in the control group one month after surgery(P<0.05).Conclusion Cluster nursing intervention can effectively reduce postoperative pain,make patients carry out rehabilitation training as soon as possible,so as to promote the functional recovery of knee joint.
