摘要
目的:分析加速康复外科(ERAS)理念指导下护理模式在全髋关节置换术后的应用效果。方法:以2020年5月至2023年6月期间入院行全髋关节置换术的患者80例为研究对象,行对比性临床研究。于术后次日将患者随机分为两组,其中采用术后常规护理模式的患者为参照组(40例),采用术后ERAS理念指导下护理模式的患者为研究组(40例)。比较两组护理前后的汉密尔顿焦虑量表(HAMA)评分、汉密尔顿抑郁量表(HAMD)评分、一般自我效能感量表(GSES)评分及Harris髋关节功能评分,并统计术后并发症发生率。结果:(1)护理前,两组的HAMA评分、HAMD评分、GSES评分比较无统计学差异(P>0.05);相较于护理前,两组护理后的HAMA评分、HAMD评分、GSES评分均改善,且研究组的HAMA评分、HAMD评分均较参照组低,GSES评分较参照组高,差异有统计学意义(P<0.05)。(2)护理前,两组的Harris髋关节功能评分比较无统计学差异(P>0.05);相较于护理前,两组护理后的Harris髋关节功能评分均升高,且研究组升高的幅度更大,差异有统计学意义(P<0.05)。(3)研究组的术后并发症发生率为7.50%,显著低于参照组的25.00%,差异有统计学意义(P<0.05)。结论:ERAS理念指导下的护理模式在全髋关节置换术后的应用效果显著,可有效改善患者的负性情绪,提升自我效能,减少术后并发症,促进髋关节功能的恢复。
Objective:To analyze the effects of the nursing model guided by the concept of accelerated rehabilitation surgery(ERAS)in patients with total hip replacement.Methods:A comparative clinical study was conducted in 80 patients who were admitted to hospital for total hip arthroplasty between May 2020 and June 2023.Patients were randomly divided into two groups on the next day after surgery,among which patients receiving conventional postoperative care mode were the reference group(40 cases),and patients receiving postoperative care mode guided by ERAS concept were the study group(40 cases).Hamilton Anxiety Scale(HAMA)score,Hamilton Depression Scale(HAMD)score,General Self-efficacy Scale(GSES)score and Harris hip function score before and after care were compared between the two groups,and the incidence of postoperative complications was analyzed.Results:(1)Before nursing,there were no significant differences in HAMA scores,HAMD scores and GSES scores between the two groups(P>0.05);Compared with before care,HAMA scores,HAMD scores and GSES scores of the two groups were improved after care,and the HAMA scores and HAMD scores of the study group were lower than those of the reference group,and GSES scores were higher than those of the reference group,with statistical significance(P<0.05).(2)Before nursing,there was no significant difference in Harris hip function scores between the two groups(P>0.05);Compared with before care,the Harris hip function score after care was increased in both groups,and the increase was greater in the study group,with statis tical significance(P<0.05).(3)The incidence of pos toperative complications in the study group was 7.50%,which was significantly lower than that in the reference group(25.00%),with statistical significance(P<0.05).Conclusion:The nursing model guided by ERAS concept has significant effects after total hip replacement,which can effectively improve patients’negative emotions,enhance self-efficacy,reduce postoperative complications,and promote the recovery of hip function.
