摘要
目的:观察利格列汀联合常规药物治疗2型糖尿病合并高血压患者的效果。方法:回顾性分析2020年12月至2022年12月该院收治的90例2型糖尿病合并高血压患者的临床资料,按照治疗方法不同将其分为观察组和对照组各45例。对照组予以常规药物(二甲双胍+氯沙坦钾+硝苯地平)治疗,观察组在对照组基础上联合利格列汀治疗,两组均治疗2个月。比较两组临床疗效,治疗前后血糖指标[糖化血红蛋白(HbA1c)、空腹血糖(FBG)、餐后2 h血糖(2hPG)]水平、血清25羟维生素D[25(OH)D]水平、空腹胰岛素(FINS)水平、胰岛素抵抗指数(HOMA-IR)水平,以及不良反应发生率。结果:观察组治疗总有效率为95.56%(43/45),高于对照组的71.11%(32/45),差异有统计学意义(P<0.05);治疗后,观察组HbA1c、FBG、2hPG、FINS、HOMA-IR水平均低于对照组,25(OH)D水平高于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:利格列汀联合常规药物治疗2型糖尿病合并高血压患者可提高治疗总有效率和25(OH)D水平,降低血糖指标水平和FINS、HOMA-IR水平,效果优于单纯常规药物治疗。
Objective:To observe effects of Linagliptin combined with conventional drugs in treatment of patients with type 2 diabetes mellitus complicated with hypertension.Methods:The clinical data of 90 patients with type 2 diabetes mellitus complicated with hypertension admitted to this hospital from December 2020 to December 2022 were retrospectively analyzed.According to different treatment methods,they were divided into observation group and control group,45 cases in each.The control group was treated with conventional drugs(Metformin+Losartan potassium+Nifedipine),while the observation group was treated with Linagliptin on the basis of that of the control group.Both groups were treated for 2 months.The clinical efficacy,the blood glucose indexes[glycosylated hemoglobin(HbA1c),fasting blood glucose(FBG),2 h postprandial blood glucose(2hPG)]levels,the serum 25-hydroxyvitamin D[25(OH)D]level,the fasting insulin(FINS)level,the insulin resistance index(HOMAIR)level,and the incidence of adverse reactions were compared between the two groups before and after the treatment.Results:The total effective rate of the observation group was 95.56%(43/45),which was higher than 71.11%(32/45)of the control group,and the difference was statistically significant(P<0.05).After the treatment,the levels of HbA1c,FBG,2hPG,FINS and HOMA-IR in the observation group were lower than those in the control group,the level of 25(OH)D was higher than that in the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Linagliptin combined with conventional drugs can improve the total effective rate and 25(OH)D level,and reduce the levels of blood glucose indexes,FINS and HOMA-IR in the patients with type 2 diabetes mellitus complicated with hypertension.Moreover,it is superior to single conventional drug therapy.
