甘肃省平原风沙与黄土丘陵地区糖尿病视网膜病变患病率调查 被引量:2

Epidemiological investigation and analysis of diabetic retinopathy in windy desert area and loess hilly area of Gansu Province

摘要 目的 探讨甘肃省平原风沙与黄土丘陵地区DR的患病率及其影响因素。方法 选取甘肃省慢性病及其危险因素监测和基本公共卫生服务管理的T2DM患者1739例,根据眼底检查结果分为单纯T2DM组(n=1328)和DR组(n=411)。比较各组一般资料及生化指标,Logistic回归分析DR的影响因素。结果 与T2DM组比较,DR组年龄、DM病程、风沙环境、吸烟史、高强度活动、FPG、HbA_(1)c、SBP、TC、HDL-C升高(P<0.05),文化程度、人均月收入、食用新鲜水果、食用坚果、步行或骑行比例降低(P<0.05)。Logistics回归分析显示,风沙环境、吸烟史、DM病程、HbA_(1)c、SBP、高强度活动是DR的危险因素,食用坚果、人均月收入是DR的保护性因素。结论 甘肃省平原风沙地区DR患病率高于黄土丘陵地区,除DM病程、HbA_(1)c、SBP、经济水平、文化程度外,环境因素可能增加DR风险。 Objective To investigate the prevalence and influencing factors of diabetic retinopathy(DR)in the windy desert areas and loess hilly areas of Gansu Province.Methods 1739 type 2 diabetes mellitus(T2DM)patients were selected for monitoring chronic diseases and their risk factors and managing basic public health services in Gansu Province.They were divided into a simple T2DM group(n=1328)and a DR group(n=411)based on fundus examination results.The general information and biochemical indicators were compared between the two group.Logistic regression analysis was used to recognize the influencing factors of DR.Results Compared with T2DM group,DR group showed an increase in age,DM duration,sandstorm environment,smoking history,high-intensity activity,FPG,HbA_(1)c,SBP,TC,and HDL-C(P<005),but decrease in educational level,per capita monthly income,fresh fruits,nuts,and the proportion of walking or cycling(P<0.05).Logistic regression analysis showed that wind-blown sand environment,smoking,DM duration,HbA_(1)c,SBP,and high-intensity activities were risk factors for DR,while nut consumption and per capita monthly income were protective factors for DR.Conclusion The prevalence of DR in the windy desert area was higher than that in the loess hilly area.In addition to the effects of DM course,HbA_(1)c,SBP,income and education level,adverse environmental factors may also increase the risk of DR.
