摘要
目的探讨提升基层卫生人员服务能力急需的培训课程,评价课程实施路径及实施效果。方法以医学教育课程开发六步法为指导,以能力为本位进行基层卫生人员培训课程开发与应用。通过问卷调查和半结构式访谈评估培训需求,确定课程目标及内容,采用线上线下混合式教学,应用线上测试、客观结构化临床考试(OSCE)进行课程评估及反馈。结果共有338名学员完成线上课程培训,课程内容综合评价成绩为(81.82~95.16分)。其中常见病与多发病诊疗模块成绩较好,疾病诊断、合理用药两个项目平均分分别是(92.16±8.46)分、(90.29±10.12)分;老年家床服务模块得分较低,老年人健康管理项目平均分(81.24±9.14)分。276名学员完成了线下实践技能的培训与考核,OSCE考核总分为(82.92±9.32)分。参加培训的338名学员普遍认为综合演练课程能够培养临床思维、团队协作能力,案例教学贴近临床,对基层工作有指导作用;OSCE能够综合考核应用能力、应急应对能力,对课程教学、考核、培训效果满意度较高。结论基于课程开发六步法设计的培训课程符合基层卫生人员工作需要,有助于提高基层诊疗水平和岗位胜任力,但也需要进一步优化和完善。
Objective To explore the training courses urgently needed to enhance the ability of primary health service personnel,and evaluate the implementation path and effect of the courses.Methods Guided by the six⁃step method of medical education curriculum development,the development and application of training courses for primary health personnel are based on ability.Questionnaire survey and semi⁃structured interview were used to assess training needs,determine course objectives and content.Application of online and offline mixed teaching,online testing and objective structured clinical examination(OSCE)for course evaluation and feedback.Results A total of 338 participants completed the online course training,and the comprehensive evaluation scores of the course content was(81.82-95.16)points.Among them,the diagnosis and treatment module of common diseases and frequently⁃occurring diseases performed better,and the average score of disease diagnosis and rational drug use were(92.16±8.46)points and(90.29±10.12)points,respectively.The score of home⁃bed service module for the elderly was relatively low,and the average score of health management items for the elderly was(81.24±914)points.276 participants completed the training and assessment of offline practical skills,and the total score of OSCE assessment was(82.92±9.32)points.The 338 trainees who participated in the training generally believed that the comprehensive exercise course could cultivate clinical thinking and teamwork abilities.And the case teaching was close to the clinic,which had a guiding role for grassroots work. OSCE can comprehensively assessthe application ability and emergency response ability. The participants were had higher satisfaction with theteaching, assessment, and training effect of the course. Conclusions The training courses for primary healthpersonnel based on the six⁃step curriculum development meet the needs of primary health personnel,which canhelp to improve the level of diagnosis and treatment and the post competence.
