摘要
目的 观察螺旋联合心电门控轴位扫描技术对于头颈部CT血管造影(CTA)图像质量和辐射剂量的影响。方法 前瞻性收集100例因疑诊头颈部血管疾病而接受头颈部CTA患者,随机分为A、B组各50例,对A组采用螺旋联合心电门控轴位扫描模式,对B组采用常规螺旋扫描模式。比较2组图像客观评价指标,包括降主动脉、颈内动脉分叉处、大脑中动脉M1段及其血管对比区域(ROC)(血管对应同层面肌肉或脑白质)的CT和标准差(SD),以及信噪比(SNR)和对比度噪声比(CNR),分析组间图像质量主观评价和辐射剂量的差异。结果 A组降主动脉SD及SD_(ROC)、颈内动脉分叉处SD_(ROC)均高于,而降主动脉SNR及CNR均低于B组(P均<0.05);组间其余客观评价差异均无统计学意义(P均>0.05)。2组图像质量均满足临床诊断要求,主观评分差异无统计学意义(P=0.878)。A组剂量长度乘积(DLP)及体型特异性剂量估计DLP(DLPSSDE)均低于B组(P均<0.001)。结论 利用螺旋联合心电门控轴位扫描技术行头颈部CTA可在保证图像质量的同时降低辐射剂量。
Objective To observe the impact of spiral combined with electrocardiographic(ECG)gated axial scanning on imaging quality and radiation dose in head and neck CT angiography(CTA).Methods Totally 100 patients who underwent head and neck CTA for suspected vascular diseases were prospectively enrolled and randomly divided into group A and B(each n=50).Spiral combined with ECG gated axial scanning was performed to collect CTA in group A,while conventional spiral scanning was performed in group B.Objective evaluation indicators of images including CT and standard deviation(SD)of descending aorta,internal carotid artery bifurcation,M1 segment of middle cerebral artery and vessels region of contrast(ROC)(muscles or white matter at the corresponding layer of vessels),as well as signal-to-noise ratio(SNR)and contrast-to-noise ratio(CNR)were compared between the two groups.The subjective scores of image quality and radiation dose were comparatively analyzed between groups.Results SD and SD ROC of descending aorta and SD ROC of internal carotid artery bifurcation were higher,while SNR and CNR of descending aorta were lower in group A than those in group B(all P<0.05).No significant difference of other objective evaluation indicators was found between the two groups(all P>0.05).The imaging quality met the clinical diagnostic requirements in both groups,and no significant difference of subjective score was found between the two groups(P=0.878).Dose-length product(DLP)and DLP calculated from size-specific dose estimates(DLP SSDE)in group A were lower than those in group B(both P<0.001).Conclusion Spiral combined with ECG-gated axial scanning applicated in head and neck CTA could maintain imaging quality while reduce radiation dose.
