冠状动脉和头颈CTA“一站式”联合扫描的可行性研究:图像质量及辐射剂量 被引量:13

Feasibility Study of One-stop Coronary Artery and Head-Neck Combined CTA Scanning: Image Quality and Radiation Dose

摘要 目的应用Revolution CT进行冠状动脉轴位扫描联合头颈血管螺旋扫描的"一站式"检查,探讨联合扫描临床应用的可行性及优势。方法纳入2016年12月至2017年3月行CTA检查的患者78位,随机分为三组,每组26例患者。A组:联合扫描,先行头颈部螺旋扫描,间隔1.1 s后切换为轴位扫描模式行心脏冠状动脉CTA扫描。B组:行自主动脉弓至颅顶的CTA扫描。C组:行自动心电门控心脏CTA扫描。记录各组CTA扫描的辐射剂量及对比剂使用量。由两位医师双盲评价三组图像质量。结果图像质量主观和客观评价结果显示,A组的头颈、心脏CTA分别与B组、C组比较均无明显差异。A组的对比剂用量(53.1±3.9)mL与B、C两组对比剂用量之和(115.2±10.6)mL相比明显减少(t=15.9,P<0.01)。A组的辐射剂量(2.1±0.4)mSv与B、C两组的辐射剂量之和(1.94±0.5)mSv相比无明显差异(t=0.09,P<0.01)。结论轴位扫描与螺旋扫描联合用于冠状动脉与头颈CTA扫描图像质量好,对比剂使用少,可用于临床心脑血管患者评价冠状动脉与头颈血管。 Objective To evaluate the feasibility and advantages of combined coronary artery axial scanning and head-neck spiral scanning with Revolution CT in clinical application.Methods A total of 78 patients who underwent CTA from December 2016 to March 2017 were randomly divided into three groups,26 patients in each group.Group A:The patients underwent combined scanning,first spiral scanning of head-neck,and then switched to axial scanning mode after 1.1 s interval for CTA scanning of coronary artery.Group B:The patients underwent CTA scanning from the aortic arch to the top of the skull.Group C:Patients underwent automatic ECG-gated cardiac CTA scanning.The radiation dose(ED)and the dosage of contrast medium in each group were recorded.The image quality of the three groups was evaluated by two doctors.Results Subjective and objective evaluation of image quality showed that there was no significant difference between group A and group B in head-neck CTA,and between group A and group C in heart CTA.The dosage of contrast medium in group A(53.1±3.9 mL)was significantly lower than that(115.2±10.6 mL)in group B and C(t=15.9,P<0.01).There was no significant difference in radiation dose between group A(2.1±0.4 mSv)and group B and C(1.94±0.5 mSv)(t=0.09,P<0.01).Conclusion Axial scanning combined with spiral scanning can be used for coronary artery and head-neck CTA scanning,which has good image quality and less contrast agent.It can be used for clinical evaluation of coronary artery and head-neck vessels in patients with cardiovascular and cerebrovascular diseases.
