摘要
目的研究比较下肢团注造影剂与上肢团注造影剂对头颈部动脉CT血管造影扫描图像的影响。方法选取2017年2月—2018年2月湖北省天门市第一人民医院收治的194例需要进行头颈部动脉CT血管造影的患者作为研究对象,按照随机数表法分为研究组、对照组,每组97例。研究组给予下肢团注造影剂,对照组则给予上肢团注造影剂,观察两组研究对象各部位血管CT值以及血管显像质量,比较分析两组研究对象头颈部动脉CT血管显像质量,并对其扫描图像质量进行评估。结果两组患者两侧颈内动脉起始部、颈内动脉入颅段、大脑中动脉起始部的CT值比较差异无统计学意义(P>0.05),观察组的颈总动脉CT值明显小于对照组(P<0.05);两组患者两侧颈总动脉、颈内动脉、大脑中动脉血管显像质量评分比较差异无统计学意义(P>0.05),而研究组两侧颈总动脉起始部、头臂干、锁骨下动脉血管显像质量评分均明显优于对照组(P<0.05);研究组两侧颈总动脉起始部、头臂干、锁骨下动脉血管显像良好,而对照组血管造影显像头臂干和锁骨下动脉显像不佳。结论在相同剂量的造影剂前提下通过下肢静脉团注造影剂,相比上肢静脉团注造影剂而言血管显像质量更好,能更直观、更立体、更清晰地显示头颈部动脉的走向、异常或病变部位,有利于头颈部血管疾病的诊断,为临床干预和治疗提供更详尽的信息,且能够减少造影剂残留所导致的不良反应,安全性高,值得临床借鉴和推广。
Objective To compare the image quality of carotid and cerebrovascular computed tomography angiography between lower extremity veins contrast medium and upper extremity veins contrast medium injection.Methods 194 patients with carotid and cerebrovascular computed tomography angiography in First People’s Hospital of Tianmen from February 2017 to February 2018 were selected and divided into two groups according to the random number table method,each with 97 cases.The study group was given lower extremity veins contrast medium injection,while the control group was given upper extremity veins contrast medium injection.Then the image quality was compared between groups.Results CT values including the origin of internal carotid artery,the cranial segment of internal carotid artery and the origin of middle cerebral artery showed no difference between the two groups(P>0.05).The CT value of common carotid artery in the observation group was significantly lower than that in the control group(P<0.05).The image quality scores of bilateral common carotid artery,internal carotid artery and middle cerebral artery showed no difference between the two groups(P>0.05),while the imaging quality scores of bilateral common carotid artery origin,brachiocephalic trunk and subclavian artery in the study group were significantly better than those in the control group(P<0.05).Conclusion Under the same dose of contrast agent,the lower extremity veins contrast medium injection in terms of upper extremity veins contrast medium injection can display the direction,abnormality or lesion of the head and neck artery more intuitively,stereoscopically and clearly,which is conducive to the diagnosis of head and neck vascular diseases,provide more detailed information for clinical intervention and treatment,and reduce the adverse reactions caused by the residual contrast agent,so it is worthy of clinical reference and promotion.
