摘要
目的探究腹腔镜下隐睾下降固定术对治疗小儿高位隐睾的临床疗效。方法选取2021年4月至2022年4月普宁市人民医院收治的高位隐睾患儿40例为研究对象,按随机数字表法分为腹腔镜组(20例)和开放组(20例)。腹腔镜组对患者行腹腔镜隐睾下降固定术进行治疗,开放组对患者行开放手术睾丸下降固定术进行治疗。比较两组的临床疗效、睾丸发育情况、手术指标、性激素[表皮生长因子(EGF)、睾酮(T)、5α-二氢睾酮(5α-DHT)、雌二醇(E_(2))、卵泡刺激素(FSH)]及并发症发生情况。结果腹腔镜组的治疗总有效率高于开放组,差异有统计学意义(P<0.05)。腹腔镜组的睾丸萎缩率及血供异常率低于开放组,差异有统计学意义(P<0.05)。腹腔镜组患儿的术中出血量少于开放组,视觉模拟评分法(VAS)评分低于开放组,下床活动时间及住院时间短于开放组,差异有统计学意义(P<0.05),两组的手术时间比较,差异无统计学意义(P>0.05)。腹腔镜组患儿治疗后的EGF、T、5α-DHT、FSH水平均高于开放组,E_(2)水平低于开放组,差异有统计学意义(P<0.05)。两组患儿术后并发症总发生率比较,差异无统计学意义(P>0.05)。结论高位隐睾患儿采用腹腔镜下隐睾下降固定术较开放手术疗效更佳,能够减少睾丸回缩和萎缩的发生,并且减少手术创伤和出血量,减轻患儿痛感,促进患儿快速康复。
Objective To explore the clinical effect of laparoscopic descend fixation of cryptorchidism on high cryptorchidism in children.Methods Forty children with high cryptorchidism admitted to the Puning People's Hospital from April 2021 to April 2022 were selected as the study objects,and they were divided into laparoscopic group(20 cases)and open group(20 cases)according to random number table method.The children in the laparoscopic group were treated with laparoscopic descendopexy of cryptorchidism,and the open group was treated with open operation descendopexy of testis.The clinical efficacy,testicular development,surgical indexes,sex hormones(epidermal growth factor[EGF],testosterone[T],5α-dihydrotestosterone[5α-DHT],estradiol[E_(2)],follicle-stimulating hormone[FSH])and complications were compared between the two groups.Results The total effective rate of laparoscopic group was higher than that of open group,and the difference was statistically significant(P<0.05).The rates of testicular atrophy and abnormal blood supply in laparoscopic group were lower than those in open group,and the differences were statistically significant(P<0.05).The intraoperative blood loss in the laparoscopic group was lower than that in the open group,the visual analogue scale(VAS)score was lower than that in the open group,and the time of getting out of bed and staying in hospital were shorter than that in the open group,with statistical significances(P<0.05),while there was no statistical significance in the operation time between the two groups(P>0.05).The levels of EGF,T,5α-DHT and FSH in the laparoscopic group after treatment were higher than those in the open group,and the levels of E_(2) were lower than those in the open group,with statistical significances(P<0.05).There was no significant difference in the total incidence of postoperative complications between the two groups(P>0.05).Conclusion Laparoscopic descent and fixation of high cryptorchidism in children has better efficacy than open surgery,which can reduce the occurr
