摘要
【目的】探讨球囊扩张联合气压弹道超声碎石术用于肾结石经皮肾镜取石(PCNL)术的价值。【方法】回顾性分析2020年3月至2022年3月宝鸡市人民医院收治的90例肾结石行PCNL术治疗的患者的临床资料,根据治疗方法的不同将其分为观察组和对照组,每组45例。观察组采用球囊扩张联合气压弹道超声碎石术,对照组采用筋膜扩张联合钬激光碎石术,比较两组围术期指标(手术时间、术中出血量、结石清除率、住院时间);比较两组手术前后血清超氧化物歧化酶(SOD)、丙二醛(MDA)、血红蛋白(Hb)、白细胞计数(WBC)水平;比较两组术后并发症发生情况。【结果】观察组手术时间短于对照组,差异有统计学意义(P<0.05)。术后,两组血清SOD、Hb水平低于术前,MDA、WBC水平高于术前,且观察组SOD、Hb水平高于对照组,MDA、WBC水平低于对照组,差异有统计学意义(P<0.05)。观察组术后并发症发生率低于对照组,差异有统计学意义(P<0.05)。【结论】球囊扩张联合气压弹道超声碎石用于肾结石PCNL的治疗中,有利于缩短手术时间,减轻应激反应,降低术后并发症发生率,具有推广价值。
【Objective】To investigate the value of balloon dilatation combined with pneumatic ballistic ultrasound lithotripsy in percutaneous nephrolithotomy(PCNL)for renal calculi.【Methods】A retrospective analysis was conducted on the clinical data of 90 patients with renal calculi who underwent PCNL from March 2020 to March 2022 in Baoji City People's Hospital.The patients were divided into observation group and control group according to different treatment methods,with 45 patients in each group.The observation group received balloon dilatation combined with pneumatic ballistic ultrasound lithotripsy,while the control group received fascial dilatation combined with holmium laser lithotripsy.Perioperative indicators(operation time,intraoperative blood loss,stone clearance rate,hospital stay)were compared between the two groups.Serum levels of superoxide dismutase(SOD),malondialdehyde(MDA),hemoglobin(Hb),and white blood cell count(WBC)were compared between the two groups before and after surgery.Postoperative complications were compared between the two groups.【Results】The observation group had a shorter operation time than the control group(P<0.05).After surgery,serum SOD and Hb levels in both groups were lower than preoperative levels,while MDA and WBC levels were higher than preoperative levels.The observation group had higher SOD and Hb levels than the control group,and lower MDA and WBC levels than the control group(P<0.05).The observation group had a lower incidence of postoperative complications than the control group(P<0.05).【Conclusion】Balloon dilatation combined with pneumatic ballistic ultrasound lithotripsy in PCNL for renal calculi can shorten the operation time,reduce stress reactions,and decrease the incidence of postoperative complications.It is worth promoting.
