摘要
骨关节炎(OA)软骨损伤修复一直是临床治疗面临的难题,目前尚无合适的药物和外科手术能够完全修复受损软骨。随着干细胞疗法的不断进步,骨髓间充质干细胞(BMSCs)在治疗软骨损伤方面体现出独特优势。BMSCs具有增殖能力强、免疫原性低及良好的成软骨分化潜能等生物学特性,现已逐步应用于OA软骨损伤的再生和修复,成为软骨组织工程中的热门种子细胞,但目前临床应用BMSCs的治疗方式较为单一。未来应加强BMSCs成软骨分化的机制研究,从生物支架材料、转化生长因子以及理化诱导环境等多方面探索高效的BMSCs成软骨转化过程,以期优化BMSCs在OA软骨损伤修复中的应用。
Repair of cartilage damage in osteoarthritis(OA)has always been a challenge for clinical treatment,and there are no existing suitable drugs or surgical procedures that can completely repair the damaged cartilage.With the continuous advancement of stem cell therapy,bone marrow mesenchymal stem cells(BMSCs)have shown unique advantages in the treatment of cartilage damage.With biological characteristics such as high proliferation capacity,low immunogenicity and favorable chondrogenic differentiation potential,BMSCs have been increasingly applied to regeneration or repair of cartilage damage in OA and have become popular seed cells in cartilage tissue engineering,but the current clinical application of BMSCs is relatively single in terms of treatment modality.In the future,the research on the mechanism of chondrogenic differentiation of BMSCs should be strengthened and the efficient process of chondrogenic transformation of BMSCs should be explored from the aspects of biological scaffolding materials,transforming growth factors and physicochemically induced environment so as to improve the application of BMSCs in the repair of cartilage damage in OA.
