摘要
目的 了解中国养老机构老年人群合理用药相关研究现状,为后续相关研究提供思路。方法 通过计算机检索PubMed、Web of Science、Embase、中国知网、万方数据库和维普期刊数据库的相关文献,对我国养老机构老年人群合理用药相关研究进行系统性检索,以评价养老机构老年人群合理用药研究现状。结果 共纳入14项研究,均为横断面研究;所有纳入的研究均在2019年之前完成,并集中在我国东部沿海地区。纳入研究的AXIS质量评分显示,得分为(11.14±2.25)分,6项研究的质量评分>10分。14项研究共调查了养老机构的5 770名老年人,涉及181家养老机构。8项研究报告了养老机构老年人群慢性病患病种类;7项研究报告了养老机构老年人用药种类。9项研究以用药依从性水平为结局,其中7项研究使用不同的分析方法对用药依从性的影响因素进行相关性分析。结果表明,年龄、受教育程度、病程、养老机构类型和服务人员关注程度等是影响养老机构老年人群用药依从性的显著性影响因素。结论 我国养老机构老年人群合理用药相关研究不足,缺乏干预性研究来评估养老机构老年人群合理用药服务的有效性;目前的研究多以横断面研究为主,且集中在用药依从性上。
OBJECTIVE To understand the current status of the research on rational drug use among the elderly population in Chinese elderly care facilities,and to provide ideas for future related research.METHODS Retrieved from PubMed,Web of Science,Embase,CNKI,Wanfang database and VIP,relevant literature about the research on rational drug use among the elderly population in Chinese elderly care facilities were collected to evaluate the current status of research on the rational drug use among the elderly population in elderly care facilities.RESULTS A total of 14 cross-sectional studies were included.All included studies were completed before 2019 and focused on the eastern coastal areas of China.The AXIS quality scores of the included studies showed a mean score of 11.14±2.25,with six studies having a quality score>10.The 14 studies examined a total of 5770 older adults in elderly care facilities,involving 181 elderly care facilities.Eight studies reported the type of chronic disease in the older adult population of the elderly care facilities;seven studies reported the type of medication used.Nine studies had medication compliance as an outcome,among which seven studies used different analytical methods to analyze the factors influencing medication compliance.The results showed that age,education,duration of illness,type of elderly care facilities,and level of staff attention had significant influence on medication compliance in the elderly population in the elderly care facilities.CONCLUSIONS There is insufficient research related to rational drug use among the elderly population in the elderly care facilities in China,and there is a lack of interventional studies to assess the effectiveness of the rational drug use service in the elderly population in the elderly care facilities.Most of the current research is based on cross-sectional studies,focusing on medication compliance.
