摘要
目的:了解养老机构的慢性病患病情况及药学服务需求,探讨适合医养结合模式的药学服务内容和方式。方法:对养老机构264名老年人进行问卷调查。结果:有95.45%的老年人患有两种或两种以上的慢性疾病,平均患病(4.38±2.23)种,患病率前5位的慢性病分别为高血压(78.79%)、白内障/青光眼(73.48%)、脑血管病(69.32%)、冠心病(47.35%)、心功能不全(39.02%)。合并用药5种及以上的老年人比例为81.82%。被调查者安全用药行为有待改善,其药学服务需求较多的分别是优化用药方案、常见药品不良反应的应对措施、漏服/忘服药品的应对措施。小册子、讲座和咨询窗口是接受度较高的药学服务开展方式。结论:养老机构的老年人群慢病共患率高,多重用药率高,对药学服务具有较高需求。药师应积极参与医养结合,开展多种药学服务,促进老年人合理用药。
Objective:Toinvestigatetheprevalence of chronic diseases and the demands of pharmaceutical care in aged care institutions and to explore the content and methods of pharmaceutical services in the mode of combination of treatment and maintenance.Methods:A questionnaire survey was conducted in264 people on aged care institutions.Results:The rate of multimorbidity(≥2 disease)was 95.45%In the cohort.average disease(4.38±2.23)species.The top 5 chronic diseases were respectivelyhypertension(78.79%),cataract/glaucoma(73.48%),cerebrovascular disease(69.32%),coronary heart disease(47.35%),and cardiac dysfunction(39.02%).The proportion of elderly patients with multi-drug(≥5)was 81.82%.The respondent's safe medication behavior needs to be improved.The top 3 needs of the pharmaceutical service are optimized drug use plan,countermeasures for common adverse drug reactions,andcountermeasures for missing/forgetting drugs.Booklets,lectures,and consultation windows were highly accepted methods of pharmaceutical services.Conclusion:The elderly population in the aged care institutions has a high rate of chronic disease,high multi-drug rate,and high demand for pharmaceutical services.Pharmacists should actively participate in the combination of treatment and maintenance,carry out a variety of pharmaceutical services,and promote the rational use of drugs for the elderly.
