摘要
目的:分析机构与家庭养老模式下慢性病老年患者用药依从性的差异及其影响因素,为健康老龄化工作的开展提供研究基础。方法:采用分层随机抽样方法,利用Morisky用药依从性量表对徐州市机构和家庭养老的老年人进行调查。结果:养老机构的慢性病老年患者用药依从率(34.3%)显著高于家庭养老(16.4%);子女个数多、有用药不良反应、没有人协助用药的慢性病老年患者用药依从性差;家庭养老模式下患病数量多、县级及以下医院就医、用药成本超过个人或家庭支付能力的慢性病老年患者用药依从性差。结论:两种养老模式下慢性病老年患者的用药依从性均不佳,且家庭养老的慢性病老年患者用药依从性更差。应加强对养老机构尤其是家庭养老的慢性病老年患者的用药指导、依从性干预教育等健康管理工作;慢性病老年患者治疗费用负担是降低用药依从性的重要因素,应重视治疗费用的医保补偿工作。
Objective:To analyze the difference and influencing factors in medication compliance of elderly patients with chronic diseases under institution care and family care modes,to provide a research basis for healthy aging work.Methods:The stratified random sampling method was used to apply Morisky medication compliance questionnaire to survey the medication compliance of elderly people in institution care and family care in Xuzhou.Results:The medication compliance rate of elderly patients with chronic diseases in institution(34.3%)was higher than family care(16.4%).Elderly patients with chronic diseases with more children,adverse reactions to drug usage,and no one to assist medication had poor medication compliance.Under the family care mode,elderly patients with chronic diseases who had a large number of illnesses,saw a doctor in hospitals at county level or below,and couldn’t afford the medication cost had poor medication compliance.Conclusion:The medication compliance of the elderly patients with chronic diseases under the two modes is not good,and the medication compliance of the elderly patients with chronic diseases under family care is worse.Health management,such as medication guidance and compliance intervention education,should be strengthened for elderly care institutions,especially for elderly patients with chronic diseases.The burden of treatment cost for elderly patients with chronic diseases is an important factor to reduce medication compliance,so medical insurance compensation for treatment cost should be paid attention to.
