摘要
临床药师参与恶性肿瘤合并肝、肾功能不全患者管理工作模式的建立及药学监护的实施,有利于保障患者用药安全、有效,改善患者的预后,突显药学服务的价值。本研究通过探索临床药师参与恶性肿瘤合并肝、肾功能不全患者药物治疗管理的工作模式及开展药学监护实践的方法与重点。初步建立临床药师参与恶性肿瘤合并肝肾功能不全患者管理的工作模式及职责。总结出药学监护重点方向包括:开展出入院药物重整服务、抗肿瘤治疗方案的制定及药物剂量的选择、肝肾功能的全程监测、不良反应的监测等。
The participation of clinical pharmacists in the establishment of the management work model for patients with malignant tumor complicated with liver and renal insufficiency and the implementation of pharmaceutical care is conducive to ensuring the safety and efficacy of drug used by the patients,improving the prognosis of patients,and highlighting the value of pharmaceutical services.In this study,we explore the working mode of clinical pharmacists in drug treatment management of patients with malignant tumors complicated with liver and renal insufficiency,and the methods and key points of pharmaceutical care practice.The working mode and responsibilities of clinical pharmacists when participating in the management of patients with malignant tumors complicated with liver and renal insufficiency were initially established.The key directions of pharmaceutical care for this special patients include providing drug reorganization services at admission and discharge,formulating anti-tumor treatment plans and selecting drug doses,wholeprocess monitoring of liver and kidney function,monitoring adverse reactions.
