摘要
目的分析后巩膜葡萄肿(PSS)边缘视网膜激光光凝在高度近视黄斑裂孔性视网膜脱离(MHRD)玻璃体切割术(PPV)中的应用效果。方法抽取2019年5月至2021年4月南阳市眼科医院收治的伴有PSS的高度近视MHRD患者126例(126只眼)作为研究对象,按随机数字表法分为对照组和观察组,每组63例(63只眼)。对照组行PPV治疗,观察组在对照组基础上采用PSS边缘视网膜激光光凝治疗。比较两组平均手术次数、视网膜复位率、最佳矫正视力(BCVA)及黄斑裂孔闭合率。结果观察组手术次数[(2.05±0.23)次]少于对照组[(2.64±0.76)次],P<0.05;观察组视网膜复位率(96.83%61/63)高于对照组(69.84%,44/63),P<0.05。术后3、6个月,观察组BCVA均低于对照组(P均<0.05)。观察组黄斑裂孔闭合率(74.60%,47/63)与对照组(61.90%,39/63)比较差异未见统计学意义(P>0.05)。结论PSS边缘视网膜激光光凝联合PPV治疗伴有PSS的高度近视MHRD,可明显减少手术次数,改善患者视力,提升视网膜复位率。
Objective To analyze the effect of posterior seleral staphyloma(PSS)marginal retinal laser photocoagulation in pars plana vitrectomy(PPV)for high myopic macular hole retinal detachment(MHRD).Methods A total of 126 high myopic MHRD patients(126 eyes)with PSS treated in Nanyang Eye Hospital from May 2019 to April 2021 were selected as the research objects,and they were divided into control group and observation group by random number table method,with 63 cases(63 eyes)in each group.The control group was treated by PPV,and the observation group was treated by PSS marginal retinal laser photocoagulation based on the treatment of the control group.The average number of operations,retinal reattachment rate,best corrected visual acuity(BCVA)and macular hole closure rate of the two groups were compared.Results The average number of operation in the observation group was(2.05±0.23)times,less than the(2.64±0.76)times in the control group(P<0.05).The retinal reattachment rate of the observation group(96.83%,61/63)was higher than that of the control group(69.84%,44/63),P<0.05.The BCVA in the study group was lower than that in the control group 3 and 6 months after surgery(all P<0.05).There was no significant difference in macular hole closure rate between the observation group(74.60%,47/63)and the control group(61.90%,39/63),P>0.05.Conclusions PSS marginal retinal laser photocoagulation combined with PPV in the treatment of high myopic MHRD complicated by PSS can significantly reduce the number of operations,improve the visual acuity of patients,improve the retinal reattachment rate.
