摘要
目的对新医改以来我国基层医疗资源配置效率和动态变化进行测度,以优化基层医疗卫生资源配置。方法分别采用DEA模型和Malmquist-DEA测算2010—2019年我国31个省份(不含港澳台)基层医疗卫生资源的配置效率、全要素生产率及其分解效率。结果各省基层医疗资源配置效率整体不高,地区间资源配置不均衡。基层医疗资源综合技术效率均值为0.726。东、中和西部综合技术效率均值分别为0.803、0.688和0.681。基层医疗资源Malmquist指数及分解指数变化均大于1;Malmquist指数排名中部(1.290)>西部(1.283)>东部(1.280)。结论提出政策建议,既要提升基层医疗资源配置效率,也要促进地区基层医疗服务均衡发展,还需加快推进基层医疗技术进步。
Objective To measure the resource allocation efficiency and dynamic changes of primary health care in China since the new health reform,so as to improve the resource allocation of primary health care.Methods DEA model and Malmquist-DEA were used to measure and analyze the allocation efficiency,total factor productivity and decomposition efficiency of primary health resources in 31 provinces(excluding Hong Kong,Macao and Taiwan)in China from 2010 to 2019.Results The allocation efficiency of primary health resources in each province was not high as a whole,and the resource allocation among regions was unbalanced.From 2010 to 2019,the average comprehensive technical efficiency value of primary health resources was 0.726,and the average comprehensive technical efficiency values of the eastern,central and western regions were 0.803,0.688 and 0.681,respectively.TheMalmquist index and decomposition index of primary health resources were all greater than 1.TheMalmquist index ranks of the middle,the west and the east were 1.290,1.283,and 1.280 in an descending order.Conclusions It is recommended to improve the allocation efficiency of primary health resources,promote the balanced development of regional primary health services,and accelerate the technical progress of primary healthcare.
