摘要
目的探讨单核细胞/高密度脂蛋白胆固醇比值(MHR)、红细胞分布宽度(RDW)、平均血小板体积(MPV)与早发冠心病(PCAD)的相关性。方法收集新疆医科大学第一附属医院在2020年6月-2021年12月接诊的326例疑似PCAD患者作为研究对象,根据冠状动脉造影(CAG)将确诊PCAD者设为观察组,非PCAD者设为对照组。比较两组临床资料,采用多因素Logistic回归分析PCAD的影响因素,Spearman相关性分析MHR、RDW、MPV与Gensini评分的关系,ROC曲线分析MHR、RDW、MPV对PCAD的诊断效能。结果共248例确诊为PCAD,其中SAP患者86例,UAP患者82例,AMI患者80例。观察组与对照组性别、年龄、高血压占比、L、PLT、TG、TC、HDL-C、UA、Cys-C比较,差异无统计学意义(P>0.05),而观察组BMI值、吸烟占比、糖尿病占比、WBC、N、M、FPG、LDL-C、Cr、Fib、MPV、RDW、MHR高于对照组,差异有统计学意义(P<0.05)。多因素Logistic回归分析显示,吸烟、WBC、RDW、MPV、MHR均是PCAD的独立危险因素(P<0.05)。不同PCAD类型患者MHR、RDW、MPV水平比较,差异有统计学意义(P<0.05)。ROC曲线分析显示,MHR、RDW、MVP诊断PCAD的曲线下面积(AUC)分别为0.813、0.781和0.699,MHR的AUC值高于MVP(P<0.05)。结论MHR、RDW、MPV与PCAD的发生密切相关,检测MHR、RDW、MPV可为早发冠心病的诊断提供参考依据。
Objective To investigate the correlation between monocyte to high density lipoprotein cholesterol ratio(MHR),red cell volume distribution width(RDW),mean platelet volume(MPV)and premature coronary artery disease(PCAD).Methods A total of 326 suspected PCAD patients admitted to the First Affiliated Hospital of Xinjiang Medical University from June 2020 to December 2021 were collected as the research objects.According to coronary angiography(CAG),those diagnosed with PCAD were set as the observation group,and those without PCAD were set as the control group.The clinical data of the two groups were compared.Multivariate Logistic regression was used to analyze the influencing factors of PCAD.Spearman correlation was used to analyze the relationship between MHR,RDW,MPV and Gensini score.ROC curve was used to analyze the diagnostic efficacy of MHR,RDW and MPV for PCAD.Results A total of 248 patients were diagnosed with PCAD,including 86 patients with SAP,82 patients with UAP,and 80 patients with AMI.There was no significant difference in gender,age,proportion of hypertension,L,PLT,TG,TC,HDL-C,UA and Cys-C between the observation group and the control group(P>0.05),while the BMI value,proportion of smoking,proportion of diabetes,WBC,N,M,FPG,LDL-C,Cr,Fib,MPV,RDW and MHR in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).Multivariate Logistic regression analysis showed that smoking,WBC,RDW,MPV and MHR were independent risk factors for PCAD(P<0.05).There were statistically significant differences in the levels of MHR,RDW and MPV among patients with different PCAD types(P<0.05).ROC curve analysis showed that the area under the curve(AUC)of MHR,RDW and MVP in the diagnosis of PCAD were 0.813,0.781 and 0.699,respectively.The AUC value of MHR was higher than that of MVP(P<0.05).Conclusion MHR,RDW and MPV are closely related to the occurrence of PCAD,and the detection of MHR,RDW and MPV can provide reference for the diagnosis of PCAD.
