幽门螺杆菌与稳定期慢性阻塞性肺疾病相关性的荟萃分析

Correlation between Helicobacter pylori and stable chronic obstructive pulmonary disease:A meta-analysis

摘要 目的:系统评价幽门螺杆菌(Hp)与稳定期慢性阻塞性肺疾病(COPD)的相关性,以期为COPD的预防及治疗提供循证依据。方法:计算机检索PubMed、Embase、Cochrane、Web of Science、CNKI、Wanfang Data、VIP等数据库,检索时限为建库至2023年3月,采用Revman 5.4软件及Stata 17.0软件进行meta分析。结果:初检675篇文献,经过筛选最终纳入19篇符合标准的文献,meta分析结果显示,COPD组的Hp阳性率显著高于健康组(OR=2.91,95%CI:2.14~3.97,P<0.001);Hp阳性的COPD患者的第1秒用力呼气容积(FEV 1)、FEV 1%pred、用力肺活量占预计值百分比(FVC%pred)、FEV 1/FVC均显著低于Hp阴性的COPD患者(MD=-0.19,95%CI:-0.26~-0.12,P<0.001)、(MD=-12.50,95%CI:-16.57~-8.42,P<0.001)、(MD=-11.22,95%CI:-14.30~-8.14,P<0.001)、(MD=-7.11,95%CI:-10.00~-4.23,P<0.001);轻度COPD患者的Hp阳性率显著低于重度COPD患者(OR=0.29,95%CI:0.15~0.54,P<0.001)。结论:当前证据显示,COPD患者较健康人群的Hp感染率更高,且影响COPD患者的肺功能及疾病严重程度。受纳入研究的数量与质量限制,上述结论还需更多前瞻性、高质量的研究进一步验证。 Objective To systematically evaluate the association between Helicobacter pylori(Hp)and stable chronic obstructive pulmonary disease(COPD),with a view to provide an evidence-based basis for the prevention and treatment of COPD.Methods Computer searches of PubMed,Embase,Cochrane,Web of Science,CNKI,Wanfang Data,and VIP databases were conducted with search dates from the date of creation to March 2023,and meta-analysis was performed using Revman 5.4 software and Stata 17.0 software.Results Initially,675 papers were retrieved.After screening,19 papers met the criteria and were included.Meta-analysis showed that the Hp positive rate was significantly higher in the COPD group than in the healthy group(OR=2.91,95%CI:2.14-3.97,P<0.001).Hp positive COPD patients had significantly lower FEV1,FEV1%pred,FVC%pred,and FEV1/FVC than in the negative group(MD=-0.19,95%CI:-0.26 to-0.12,P<0.001),(MD=-12.50,95%CI:-16.57 to-8.42,P<0.001),(MD=-11.22,95%CI:-14.30 to-8.14,P<0.001),(MD=-7.11,95%CI:-10.00 to-4.23,P<0.001).The mild COPD patients had significantly lower Hp positivity than the severe COPD(OR=0.29,95%CI:0.15-0.54,P<0.001).Conclusions Current evidence suggests that Hp infection is more prevalent in COPD patients than in the healthy population and affects lung function and disease severity in COPD patients.Due to the restricted number and quality of the included studies,these findings need further validation by more prospective and high-quality studies.
