磁共振动态增强诊断乳腺良恶性病变的准确性分析 被引量:3

摘要 目的:探究磁共振(MR)动态增强诊断乳腺良恶性病变的准确性。方法:选取2020年8月-2022年9月中国人民解放军联勤保障部队第九一〇医院收治的91例乳腺病变患者作为研究对象,均对其实施高频超声、MR动态增强扫描检查,以病理学诊断结果为金标准,比较两种检查方法对乳腺良恶性病变的诊断效能。结果:MR动态增强扫描诊断乳腺恶性病变的特异度、敏感度、阳性预测值、阴性预测值和准确率均高于高频超声诊断,差异有统计学意义(P<0.05)。乳腺恶性病变患者高频超声显示乳腺肿块外形不规则,呈蟹足状或星状,与周围组织缺乏明显界限,内部回声欠均匀,伴微小钙化,血流信号分级多为2级、3级;MR动态增强扫描显示病灶信号呈长T1、长T2信号,强弱不等,边界模糊,呈分叶状、毛刺状,不规则形态,增强扫描后出现不同程度强化。恶性病变患者MR动态增强扫描早期强化率高于良性病变患者,差异有统计学意义(P<0.05),但良性病变患者与恶性病变患者峰值强化率比较,差异无统计学意义(P>0.05)。结论:在乳腺良恶性病变鉴别诊断中,实施MR动态增强扫描可提高诊断特异度、敏感度,为临床有效诊治该疾病提供可靠依据。 Objective:To explore the accuracy of magnetic resonance(MR)dynamic enhancement in diagnosis of benign and malignant breast lesions.Method:A total of 91 patients with breast lesions admitted to the 910th Hospital of the Joint Logistics Support Force of the People's Liberation Army from August 2020 to September 2022 were selected as the research subjects,and all of them were subjected to high-frequency ultrasound and MR dynamic enhancement scanning,and the pathological diagnosis results were used as the gold standard,the diagnostic efficacy of the two examination methods for benign and malignant breast lesions was compared.Result:The specificity,sensitivity,positive predictive value,negative predictive value and accuracy of MR dynamic enhancement scanning for the diagnosis of malignant breast lesions were higher than those of high-frequency ultrasound diagnosis,the differences were statistically significant(P<0.05).High-frequency ultrasound in patients with malignant breast lesions shows that the appearance of the breast mass is irregular,in the shape of crab feet or stars,lacking clear boundaries with surrounding tissues,with uneven internal echoes and accompanied by small calcifications,the blood flow signal grading is mostly level 2,level 3.MR dynamic enhancement scanning showed that the lesion signal presented as long T1 and long T2 signals,with varying intensities,blurred boundaries,lobulated,spiculated,and irregular shapes.After enhanced scanning,there were varying degrees of enhancement.The early enhancement rate of MR dynamic enhancement in patients with malignant lesions was higher than that in patients with benign lesions,the difference was statistically significant(P<0.05),but there was no significant difference in peak enhancement rate between patients with benign and malignant lesions(P>0.05).Conclusion:In the differential diagnosis of benign and malignant breast lesions,implementing MR dynamic enhancement can improve diagnostic specificity,sensitivity,providing reliable basis for effective clinical di
