摘要
目的:探讨分析MR动态增强扫描对乳腺病变患者的应用价值。方法:回顾性分析2020年1月-2022年1月佛山市三水区人民医院收治的146例乳腺病变患者的临床资料,均进行乳腺MR动态增强扫描及超声检查,且经临床及病理证实为乳腺病变,以病理结果为“金标准”,分为良性病变组(n=68)、恶性病变组(n=78)。记录MR动态增强扫描和超声检查乳腺良、恶性病变检出率,比较MR动态增强扫描检查与超声检查的准确度、特异度、敏感度。分析MR动态增强扫描的时间强度曲线(TIC曲线),并比较两组MR动态增强扫描早期强化率与峰值强化率。结果:MR动态增强扫描检查良性病变检出率为95.59%,恶性病变检出率为93.59%,均高于超声检查的85.29%、83.33%,差异有统计学意义(χ^(2)=4.168、4.019,P=0.041、0.045)。MR动态增强扫描诊断结果准确度、特异度、敏感度均高于超声检查结果,差异有统计学意义(P<0.05)。良性病变组的TIC曲线以Ⅰ型为主;恶性病变组的TIC曲线以Ⅲ型为主,MR动态增强扫描检查两组的TIC曲线类型比较,差异有统计学意义(P<0.05)。两组MR动态扫描峰值强化率比较,差异无统计学意义(P>0.05),恶性病变组MR动态增强扫描早期强化率显著高于良性病变组,差异有统计学意义(P<0.05)。结论:乳腺病变患者采用MR动态增强扫描进行鉴别诊断,可提高鉴别良、恶性病变的准确率,从而帮助临床医师更好地为患者制订有效、适宜的治疗方案。
Objective:To explore and analyze the application value of MR dynamic enhancement scanning in patients with breast lesions.Method:A retrospective analysis was conducted on 146 patients with breast lesions admitted to Foshan Sanshui District People's Hospital from January 2020 to January 2022.All patients underwent MR dynamic enhancement scanning and ultrasound examination,and were clinically and pathologically confirmed as breast lesions.Based on pathological results as the"gold standard",they were divided into benign lesion group(n=68)and malignant lesion group(n=78).Record the detection rates of benign and malignant breast lesions using MR dynamic enhancement scanning and ultrasound examination,and compared the accuracy,specificity,and sensitivity of MR dynamic enhancement scanning and ultrasound examination.Analyzed the time intensity curve(TIC curve)of MR dynamic enhancement scanning and compared the early enhancement rate and peak enhancement rate of two groups of MR dynamic enhancement scanning.Result:The detection rate of benign lesions and malignant lesions of MR dynamic enhancement scanning was 95.59%and 93.59%,both higher than 85.29%and 83.33%of ultrasound examination,the differences were statistically significant(χ^(2)=4.168,4.019,P=0.041,0.045).The accuracy,specificity,and sensitivity of MR dynamic enhancement scanning were higher than those of ultrasound examination,and the differences were statistically significant(P<0.05).The TIC curve of the benign lesion group is mainly typeⅠ;the TIC curve of the malignant lesion group is mainly typeⅢ,and there is statistically significant difference in the type of TIC curve between the two groups on MR dynamic enhancement scanning(P<0.05).There was no statistically significant difference in the peak enhancement rate between the two groups of MR dynamic enhancement scanning(P>0.05).The early enhancement rate of MR dynamic enhancement scanning of the malignant lesion group was significantly higher than that of the benign lesion group,the difference was statist
