Effect of early rehabilitation on neurodevelopment and swallowing function in children with severe craniocerebral trauma and dysphagia

摘要 目的探讨早期康复对重度颅脑外伤吞咽困难患儿神经发育及吞咽功能的影响。方法选取2022年1月至10月湖南省儿童医院收治的重度颅脑外伤吞咽困难患儿84例,采取随机数字表法将其分为对照组(常规治疗)和观察组(常规治疗配合早期康复治疗),每组42例,两组均治疗4周。采用洼田饮水试验和吞咽困难调查量表(DDS)、身高别体重Z评分(WHZ)评定两组治疗前后吞咽困难程度、营养状况,并采用粗大运动功能评定量表88项(GMFM-88)、Gesell发育诊断量表发育商(DQ)评定两组治疗前后神经发育情况。结果治疗后,两组洼田饮水试验、DDS第二部分评分均低于治疗前,且观察组低于对照组(P<0.05)。治疗后,两组WHZ、GMFM-D区评分、GMFM-E区评分、粗大运动DQ、精细运动DQ均高于治疗前,且观察组高于对照组(P<0.05)。结论早期康复治疗对重度颅脑外伤吞咽困难患儿效果显著,能有效改善其营养状况,促进神经发育及吞咽功能恢复。 Objective To investigate the effect of early rehabilitation on neurodevelopment and swallowing function in children with severe craniocerebral trauma and dysphagia.Methods A total of 84 children with severe craniocerebral trauma and dysphagia admitted to Hunan Provincial Children’s Hospital from January to October 2022 were selected and divided into control group(conventional treatment)and observation group(conventional treatment combined with early rehabilitation treatment)by random number table method,42 cases in each group.Both groups were treated for four weeks.The dysphagia degree and nutritional status of the two groups before and after treatment were assessed by the Kubota water swallowing test,dysphagia disorders survey scale(DDS),weight for height Z score(WHZ),and the neurodevelopment of the two groups was assessed by gross motor function measure-88(GMFM-88)and developmental quotient(DQ)of Gesell developmental diagnostic scale.Results After treatment,the scores of Kubota water swallowing test and part two of DDS in two groups were lower than those before treatment,and those of observation group were lower than those of control group(P<0.05).After treatment,WHZ,GMFM-D part score,GMFM-E part score,gross motor DQ,and fine motor DQ in two groups were higher than those before treatment,and those of observation group were higher than those of control group(P<0.05).Conclusion Early rehabilitation therapy has significant effect on children with severe craniocerebral trauma and dysphagia,which can effectively improve nutritional status and promote the recovery of neurodevelopmental swallowing function.
