摘要
目的 系统评价西帕依固龈液联合西吡氯铵含漱液治疗口腔念珠菌病的有效性和安全性,为临床决策提供循证医学依据。方法 计算机全面检索Cochrane Library、Pub Med、EMBASE、Web of Science、Sino Med、VIP、Wanfang、CNKI数据库。检索自建库至2022年10月发表的西帕依固龈液联合西吡氯铵含漱液治疗口腔念珠菌病的随机对照试验研究(RCTs)。对纳入的文献进行资料提取与质量评价,使用Revman 5.4软件进行Meta分析。结果 纳入12项RCTs,共1 220例口腔念珠菌病患者。Meta分析结果显示,与对照组相比,西帕依固龈液联合西吡氯铵含漱液治疗口腔念珠菌病能提高有效率[OR=4.38,95%CI(2.96,6.49),P <0.00001]、疼痛改善率[WMD=-0.55,95%CI(-0.58,-0.52),P <0.00001]、黏膜萎缩改善率[WMD=-0.39,95%CI(-0.43,-0.36),P <0.000 01]、黏膜红斑改善率[WMD=-0.57,95%CI(-0.62,-0.53),P <0.000 01]、汉密尔顿抑郁量表(HAMD)评分改善率[WMD=-12.22,95%CI(-17.69,-6.75),P <0.000 1]和汉密尔顿焦虑量表(HAMA)评分改善率[WMD=-7.97,95%CI(-8.53,-7.42),P <0.000 01],在不良反应发生率方面,差异无统计学意义[OR=0.92,95%CI(0.52,1.63),P=0.77]。结论 西帕依固龈液联合西吡氯铵含漱液治疗口腔念珠菌病可提高患者生活质量,尚无减少不良反应的发生的证据。由于纳入研究的数量和质量有限,该结论尚需开展更多高质量的研究加以证实。
AIM To systematically evaluate the efficacy and safety of Xipayi gingival solid fluid combined with cilpyrammonium chloride gargle in the treatment of oral Candidiasis,and to provide evidence-based medical evidence for the clinical treatment of oral Candidiasis.METHODS Eight databases including Cochrane Library,PubMed,EMBASE,Web of Science,SinoMed,VIP,Wanfang and CNKI were searched by computer.Randomized controlled trials(RCTs)of Xipayi gingival solid fluid combined with cilpyrammonium chloride gargle in the treatment of oral Candidiasis were retrieved from the establishment of the database to October 2022.Data extraction and quality evaluation were performed for the included literatures,and Meta-analysis was performed using Revman 5.4 software.RESULTS A total of 1 220patients with oral Candidiasis were included in 12 RCTs.The results of Meta-analysis showed that,compared with the control group,the treatment of oral Candidiasis with Xipayi gingival solid fluid combined with cilpyrammonium chloride gargle could improve the effective rate [OR=4.38,95%CI(2.96,6.49),P<0.000 01],and reduce pain score(WMD=0.55,95%CI(0.58,0.52),P<0.00001],mucosa atrophy scores[WMD=0.39,95%CI(0.43,0.36),P<0.000 01),mucosal erythema score [WMD=0.57,95%CI(0.6 2,0.53),P<0.000 01],HAMD scores[WMD=12.22,95%CI(17.69,6.75),P<0.000 1],HAMA scores[WMD = 7.97,95% CI(8.53,7.42),P<0.000 01].The incidence of adverse reactions had no statistical significance[OR=0.92,95%CI(0.52,1.63),P=0.77].CONCLUSION The curative effect of Xipayi gingival solid fluid combined with cilpyrammonium chloride gargle in the treatment of oral Candidiasis is better than that of the control group,and can improve the quality of life of patients,but can not reduce the occurrence of adverse reactions.Due to the limited number and quality of included studies,more high-quality studies are needed to confirm the above conclusions.
