磁共振成像增强扫描判定鼻咽癌根治性放射治疗后的疗效研究

摘要 目的:探讨磁共振成像(MRI)增强扫描在判定鼻咽癌(NPC)根治性放射治疗后疗效的研究。方法:选取医院收治的83例NPC患者,所有患者均接受放射治疗,且分别在放射治疗前后进行MRI增强扫描,分析并记录其影像学结果。结果:接受放射治疗前MRI影像学检查的83例患者肿瘤病灶T2加权成像(T2WI)出现不均匀的高信号等表现,T1加权成像(T1WI)表现出较低的信号,而在增强MRI扫描之后对肿瘤病灶的形状、形态以及周围部位进行观察,其中54例患者病灶侵犯到翼内肌,15例患者病灶侵犯到翼外肌,14例患者病灶侵犯到口咽部;同时有67例患者的颈部淋巴结出现不同程度的肿大。83例患者在接受1~6个疗程的放射治疗后,再次接受MRI增强扫描,其中78例患者的肿瘤病灶范围明显缩小,肿瘤病灶的边界相对较为清晰,对周围组织侵犯的范围也逐渐缩小,已经出现肿大的淋巴结开始有所缩小,放射治疗前与放射治疗后相比,其余5例患者放射治疗前后实施MRI检查并未发现明显的改变。83例患者经过放射治疗前后的肿瘤病灶以及淋巴结MRI检查结果显示,放射治疗后与放射治疗前相比,肿瘤病灶以及淋巴结直径、弥散系数(ADC)值均缩短,且随着放射治疗剂量的增加,肿瘤病灶以及淋巴结直径、ADC值不断缩短,经过重复测量方差分析可见,放射治疗前(0 Gy)和放射治疗时(30 Gy、50 Gy及70 Gy)不同剂量下肿瘤病灶及淋巴结直径比较,差异均有统计学意义(F=284.094,F=51.313;P<0.001);放射治疗前(0 Gy)和放射治疗时(30 Gy、50 Gy及70 Gy)不同剂量下肿瘤病灶及淋巴结直径的ADC值比较,差异均有统计学意义(F=3.361,F=15.108;P<0.05)。83例患者实施放射治疗后行MRI增强扫描可见,病灶边界、侵犯口咽、侵犯翼内肌、侵犯翼外肌以及淋巴结肿大的病灶周围侵犯发生率分别为6.02%、12.05%、24.10%、12.05%和3.61%。结论:对NPC患者实施MRI增强扫描能够充 Objective:To explore and analyze the application value of enhanced magnetic resonance imaging(MRI)scan in judging the curative effect of radical radiation therapy for nasopharyngeal carcinoma(NPC).Methods:A total of 83 NPC patients admitted to hospital were selected and all patients received radiotherapy,and enhanced MRI scans were respectively performed on them before and after radiotherapy.The imaging results of them were recorded and analyzed.Results:The results of MRI scanning examination of 83 patients before received radiotherapy indicated that T2 weighted imaging(T2WI)of tumor lesions occurred unevenly high signals and so on,and T1WI appeared lower signals.The observations on the shape,morphology and surrounding areas after enhanced MRI scan indicated the lesions of 54 cases had invaded to the medial pterygoid muscle,and the lesions of 15 cases had invaded to the lateral pterygoid muscle,and the lesions of 14 cases had invaded to the oropharyngeal region,and the cervical lymph nodes of 67 patients occurred the swelling with different degrees.83 patients received enhanced MRI scans again after they received 1 to 6 courses of radiotherapy.The range of tumor lesions of 78 cases of them obviously reduced,and the boundaries of tumor lesions of them were relatively clear,and the invaded range to surrounding tissue also gradually shrunk,and the lymph node that had occurred swollen started to shrink.The compared results between before and after radiotherapy indicated that the MRI examinations of other 5 patients before and after radiotherapy did not find obvious changes.The results of MRI examination of tumor lesions and lymph nodes in 83 patients before and after radiotherapy showed that the diameters of tumor lesions and lymph node,and apparent diffusion coefficient(ADC)shortened after radiotherapy than those before radiotherapy,and the diameters of them and the ADC values were continuously shortening with the increasing of radiation dose.The results of variance analysis of repeated measurement indicated that th
