摘要
目的探讨人工髋关节置换术(HA)治疗老年股骨粗隆间骨折的效果。方法回顾性分析2017年1月—2020年3月我院治疗的88例老年股骨粗隆间骨折患者,根据手术方式不同,将采用股骨近端防旋髓内钉(PFNA)内固定术治疗的患者设为PFNA组,将采用HA治疗的患者设为HA组,每组各44例。比较两组围术期情况、治疗效果及并发症发生率。结果PFNA组手术时间、术中出血量、切口长度均少于HA组,下床时间、负重时间均长于HA组,两组比较差异均有统计学意义(t=8.883、14.844、13.089、18.993、30.150,均P<0.05);而两组治疗效果比较,差异无统计学意义(χ^(2)=0.848,P>0.05);HA组术后并发症发生率显著低于PFNA组,两组比较差异有统计学意义(χ^(2)=5.091,P<0.05)。结论对老年股骨粗隆间骨折患者开展HA、PFNA内固定术均有较好的治疗效果,两种方式各有优势。
Objective To explore the clinical effect of hip arthroplasty(HA)in the treatment of femur intertrochanteric fractures in the elderly.Methods A retrospective analysis of 88 elderly with intertrochanteric fractures of the femur who received treatment from January 2017 to March 2020 in our hospital.According to different surgical procedures,the patients who received proximal femoral nail anti-rotation(PFNA)internal fixation were set as the PFNA group,and the patients who received HA were set as the HA group,with 44 cases in each group.The surgical conditions,treatment results and complication rates of the two groups were compared.Results The operation time,intraoperative bleeding,and incision length in the PFNA group were less than those in the HA group,and the time to get out of bed and weight-bearing time were longer than those in the HA group,and the differences were statistically significant between the two groups(t=8.883,14.844,13.089,18.993,30.150,all P<0.05);while the differences were not statistically significant when comparing the treatment effects of the two groups(x?=0.848,P>0.05);the incidence of postoperative complications in the HA group was significantly lower than that in the PFNA group,and the difference between the two groups was statistically significant(x*=5.091,P<0.05).Conclusion Both HA and PFNA internal fixation for elderly patients with femoral intertrochanteric fractures have good therapeutic effects,and the two methods have their own advantages.
