摘要
目的:探讨股骨粗隆间骨折股骨近端防旋髓内钉(PFNA)术后联合使用恒古骨伤愈合剂的临床疗效及可能机制。方法:2017年1月—2020年6月在我院收治的股骨粗隆间骨折患者70例,均由同一医生主刀完成PFNA手术,术后给予常规抗凝剂及预防感染治疗。根据术后口服药物的不同将患者分为观察组和对照组,观察组术后口服恒古骨伤愈合剂4周,对照组服用桃红四物汤治疗4周,比较2组患者髋关节肿胀消退时间、瘀斑消退时间、骨折愈合时间;术后即刻以及术后4周比较血清中成骨相关实验指标;术后3个月比较髋关节Harris评分。结果:观察组在髋关节肿胀消退时间、瘀斑消退时间以及骨折愈合时间上均短于对照组(P<0.05);术后即刻观察组与对照组血清中成骨相关指标碱性磷酸酶(ALP)、骨钙素(BGP)及Ⅰ型前胶原羧基末端前肽(PICP)比较差异无统计学意义(P>0.05);术后4周,观察组血清中ALP、BGP指标高于对照组(P<0.05),PICP低于对照组(P<0.05);术后3个月观察组Harris评分高于对照组(P<0.05)。结论:恒古骨伤愈合剂用于股骨粗隆间骨折PFNA术后,可促进患髋消肿、瘀斑消失,加快骨折愈合速度及髋关节功能恢复,其疗效明显优于桃红四物汤,作用机理可能与提高血清中ALP和BGP含量、降低PICP含量,促进成骨过程相关。
Objective:To investigate the clinical efficacy and possible mechanisms of using Henggu Gushang Healing Agent combination with Proximal Femoral Nail Antirotation(PFNA)for Intertrochanteric fracture of femur.Methods:A total of 70 patients with Intertrochanteric fracture of femur treated with PFNA by the same surgeon from January 2017 to June 2020 were included.Routine anticoagulants and infection prevention treatments were given postoperatively.Patients were divided into observation group and control group according to different oral medications after surgery.The observation group received Henggu Gushang Healing Agent for 4 weeks,while the control group received Taohong Siwu Decoction for 4 weeks.The time of hip joint swelling disappearance,bruising disappearance,fracture healing,serum bone-related experimental indicators were compared between the two groups immediately after surgery and at 4 weeks postoperatively.Harris score of the hip joint was compared at 3 months postoperatively.Results:The time of hip joint swelling disappearance,bruising disappearance,and fracture healing in the observation group was shorter than that in the control group(P<0.05).There was no statistically significant difference in serum bone-related indicators such as alkaline phosphatase(ALP),bone gla protein(BGP),and type I procollagen carboxy-terminal peptide(PICP)between the observation group and the control group immediately after surgery(P>0.05).At 4 weeks postoperatively,the serum ALP and BGP levels in the observation group were higher than those in the control group(P<0.05),while the PICP level was lower than that in the control group(P<0.05).The Harris score of the observation group was higher than that of the control group at 3 months postoperatively(P<0.05).Conclusion:Henggu Gushang Healing Agent can promote the disappearance of swelling and bruising in the hip joint,accelerate the healing of fractures,and improve the recovery of hip joint function after PFNA surgery for Intertrochanteric fracture of femur.Its function is signifi
