摘要
目的探讨人工髋关节置换术与股骨近端髓内钉内固定治疗高龄股骨粗隆间骨折的临床效果。方法选择2017年3月至2020年3月我院收治的70例高龄股骨粗隆间骨折患者,采用随机数字表法分为两组,每组35例。对照组行股骨近端髓内钉内固定,观察组行人工髋关节置换术。对两组临床指标、髋关节功能评分,并发症进行比较。结果观察组手术时间(51.09±7.74)min、术后下床活动时间(7.36±1.27)d、负重时间(22.98±4.59)d,均短于对照组(78.42±12.45)min、(39.78±6.35)d、(75.58±10.67)d,出血量(87.45±9.22)m L、并发症发生率(5.71%)低于对照组(119.78±12.53)m L、(25.71%),髋关节功能评分(88.82±7.38)分高于对照组(79.59±6.72)分,差异有统计学意义(P<0.05)。结论与股骨近端髓内钉内固定相比,高龄股骨粗隆间骨折行人工髋关节置换术效果更好,手术时间短,创伤小,出血少,有利于患者术后尽早展开运动,并发症少,髋关节功能恢复快。
Objective This paper aims to investigate the clinical effect of artificial hip replacement and proximal femoral nail internal fixation in the treatment of intertrochanteric fractures of the femur.Methods 70 elderly patients with intertrochanteric fractures of the femur who were admitted to our hospital from March 2018 to March 2020 were selected and divided into two groups by random number table,with 35 cases in each group.The control group received intramedullary nail fixation of the proximal femur,and the observation group received artificial hip replacement.The clinical indicators,hip function scores,and complications were compared between the two groups.Results The operation time of the observation group was(51.09±7.74)min,the postoperative time to get out of bed(7.36±1.27)d,and the weight-bearing time(22.98±4.59)d were shorter than those of the control group(78.42±12.45)min and(39.78±6.35)d,(75.58±10.67)d,blood loss(87.45±9.22)m L,complication rate(5.71%)lower than the control group(119.78±12.53)m L,(25.71%),hip joint function score(88.82±7.38)Points were higher than the control group(79.59±6.72)points,the difference was statistically significant(P<0.05).Conclusion Compared with the internal fixation of the proximal femur with intramedullary nails,artificial hip replacement for intertrochanteric fractures of the elderly is more effective,with shorter operation time,less trauma,and less bleeding,which is beneficial to the early postoperative exercise and fewer complications,the hip function recovers better and faster.
