摘要
目的:评估在肺结核临床诊断中应用GeneXpert MTB/RIF(结核分枝杆菌/利福平耐药检测)的价值,为肺结核疾病诊断、治疗工作提供参考。方法:评估对象选择2020年3月—2021年3月天津市滨海新区塘沽传染病医院就诊的60例肺结核疑似与确诊患者,对患者痰标本进行收集,实行GeneXpert MTB/RIF、抗酸染色法、固体培养实验检测,比较三种检测方法耐药菌和结核杆菌的阳性率;并比较GeneXpert MTB/RIF、抗酸染色法的特异性和灵敏度。结果:相比固体培养法,GeneXpert MTB/RIF的实行,阳性检出率提升明显,差异有统计学意义(χ^(2)=4.3854,P<0.05);与固体培养法相比,抗酸染色法的实行,阳性检出率降低明显,差异有统计学意义(χ^(2)=4.0615,P<0.05);与抗酸染色法相比,GeneXpert MTB/RIF的实行,特异性和灵敏度均明显提升,差异有统计学意义(χ^(2)=17.7882,6.8056,P<0.05)。结论:在肺结核临床诊断中应用GeneXpert MTB/RIF价值较高,特异性、灵敏度和准确性均较高,操作方便,有助于疾病的早期治疗工作。
Objective:To evaluate the value of GeneXpert MTB/RIF(Mycobacterium tuberculosis/rifampicin resistance test)in the clinical diagnosis of pulmonary tuberculosis and to provide a reference for the diagnosis and treatment of pulmonary tuberculo-sis disease.Methods:60 patients with suspected and confirmed pulmonary tuberculosis attending hospitals from March 2020 to March 2021 were selected for evaluation.Sputum specimens were collected from patients and tested by GeneXpert MTB/RIF,antac-id staining method and solid culture assay.Positive rates of drug-resistant bacteria and Mycobacterium tuberculosis were compared among the three detection methods,and the specificity and sensitivity of the GeneXpert MTB/RIF and antacid staining methods were compared.Results:Compared with the solid culture method,the positive detection rate of GeneXpert MTB/RIF was signifi-cantly improved,and the difference was statistically significant(χ^(2)=4.3854,P<0.05).Compared with the solid culture method,the positive detection rate of the antacid staining method was reduced significantly and the difference was statistically significant(χ^(2)=4.0615,P<0.05).Compared with the antacid staining method,the specificity and sensitivity of GeneXpert MTB/RIF were signifi-cantly improved,and the difference was statistically significant(χ^(2)=17.7882,6.8056,P<0.05).Conclusion:The application of GeneXpert MTB/RIF in the clinical diagnosis of pulmonary tuberculosis is of high value,with high specificity,sensitivity and accu-racy,easy to operate,and contributes to the early treatment work of the disease.
