摘要
目的评估单核细胞/高密度脂蛋白比值(MHR)、中性粒细胞与淋巴细胞比值(NLR)及γ-谷氨酰基转移酶与血小板比值(GPR)在不稳定型心绞痛(UAP)患者中的水平及与冠状动脉病变程度的关系。方法分析2021年6月-2022年12月长沙市第三医院心血管内科收治UAP患者110例为UAP组,另选取同期医院健康体检者95例为健康对照组。比较2组及不同冠状动脉狭窄程度UAP患者MHR、NLR、GPR水平,Pearson检验分析MHR、NLR、GPR与冠状动脉狭窄程度的相关性,绘制受试者工作特征曲线(ROC)检验三者对UAP冠状动脉狭窄程度的预测效果。结果UAP组MHR、NLR及GPR均高于健康对照组(t=25.833、68.758、20.684,P均<0.001)。不同Gensini评分患者MHR、NLR及GPR水平比较,轻度亚组<中度亚组<重度亚组(t=61.420、750.880、154.810,P均<0.001)。MHR、NLR、GPR与Gensini评分均呈正相关(r=0.702、0.657、0.669,P均<0.001)。MHR、NLR、GPR及三者联合预测UAP冠状动脉狭窄程度的曲线下面积(AUC)分别为0.901、0.868、0.863、0.921,三者联合的AUC大于单项检测(P<0.001)。结论UAP患者中MHR、NLR及GPR异常升高,且与Gensini评分具有一定相关性,可能成为判断UAP发生、评估病变程度的重要指标。
Objective To evaluate the ratio of Monocyte to High-density lipoprotein(MHR),neutrophil to lymphocyte(NLR)andγ-The expression of glutamyltransferase to platelet ratio(GPR)in patients with unstable angina pectoris(UAP)and its relationship with the degree of coronary artery disease.Methods Retrospective analysis of 110 UAP patients admitted to the Cardiovascular Department of the Third Hospital of Changsha from June 2021 to December 2022 was conducted as the UAP group,and 95 health examinees in the same period were selected as the healthy control group.Compare the levels of MHR,NLR,and GPR between two groups and patients with different degrees of coronary artery stenosis in UAP.Pearson test is used to analyze the correlation between MHR,NLR,and GPR and the degree of coronary artery stenosis.The receiver operating characteristic curve(ROC)is drawn to test the predictive effect of the three on the degree of coronary artery stenosis in UAP.Results The MHR,NLR,and GPR of the UAP group were higher than those of the healthy control group(t=25.833,68.758,20.684,P<0.001).Comparison of MHR,NLR,and GPR levels among patients with different Gensini scores,mild subgroup<moderate subgroup<severe subgroup(t=61.420,750.880,154.810,P<0.001).MHR,NLR,GPR were all positively correlated with Gensini scores(r=0.702,0.657,0.669,P<0.001).The area under the curve(AUC)of MHR,NLR,GPR,and their combination in predicting the degree of coronary artery stenosis in UAP were 0.901,0.868,0.863,and 0.921,respectively.The combined AUC of the three was higher than single index detection(P<0.001).Conclusion Abnormal elevation of MHR,NLR,and GPR in patients with UAP,which is correlated with the Gensini score,may become an important indicator for judging the occurrence of UAP and evaluating the degree of lesions.
