摘要
目的探讨三维运动平台康复训练结合针刺治疗中风后膝过伸的疗效。方法选择94例中风后膝过伸患者,随机分为对照组与观察组,各47例。对照组采用普通针刺进行治疗,观察组在对照组的基础上联合三维运动平台康复训练进行治疗,2组均治疗4周。比较2组临床疗效、下肢肌力水平、步行功能、运动功能及膝过伸次数。结果治疗4周后,观察组总有效率(91.49%,43/47)高于对照组(74.47%,35/47)(P<0.05);与治疗前比较,2组伸肌峰力矩、屈肌峰力矩、屈伸肌峰力矩、峰力矩体质量比值、步频、步速、步幅及Fugl-Meyer评测法(FMA)下肢、Berg平衡量表(BBS)评分升高,观察组高于对照组(P<0.05)。2组起立-行走计时测试(TUGT)、10 m步行测试(10 MWT)缩短,观察组短于对照组(P<0.05);2组改良Ashworth量表(MAS)评分、膝过伸次数降低,观察组低于对照组(P<0.05)。结论三维运动平台康复训练结合针刺治疗中风后膝过伸,可有效改善患者下肢运动功能,减少患者膝过伸次数,临床疗效较好。
Objective To study the therapeutic effect of three-dimensional exercise platform rehabilitation training combined with acupuncture on knee hyperextension after stroke.Methods 94 patients with knee hyperextension after stroke were selected and randomly divided into the control group and the observation group,with 47 patients in each group.The control group was treated with ordinary acupuncture,and the observation group was treated with three-dimensional exercise platform rehabilitation training on the basis of the the treatment of the control group.Both groups were treated for 4 weeks.The therapeutic effect,lower limb muscle strength level,walking function,motor function and knee hyperextension times were compared between the two groups.Results After 4 weeks of treatment,the total effective rate of the observation group(91.49%,43/47)was higher than that of the control group(74.47%,35/47)(P<0.05).After 4 weeks of treatment,compared with before treatment,the peak moment of extensor,peak moment of flexor,peak moment/weight ratio of flexor and extensor,step frequency,pace,stride length,scores of Fugl-Meyer assessment(FMA)lower limbs and Berg balance scale(BBS)of the two groups increased,and the observation group was higher than the control group(P<0.05).After 4 weeks of treatment,Timed Up and Go Test(TUGT)and 10-Meter Walk Test(10 MWT)of the two groups were shortened,and the observation group was shorter than the control group(P<0.05).After 4 weeks of treatment,the scores of modified Ashworth scale(MAS)and knee hyperextension times of the two groups decreased,and the observation group was lower than the control group(P<0.05).Conclusion Three-dimensional exercise platform rehabilitation training combined with acupuncture in the treatment of knee hyperextension after stroke can effectively improve the lower limb motor function of patients,reduce the number of knee hyperextension of patients,and has good clinical efficacy.
