摘要
目的:探究体重指数(BMI)对慢性阻塞性肺疾病急性加重期(AECOPD)患者住院时间的影响。方法:选取2007年10月—2022年10月海安市中医院肺病科收治的AECOPD患者212例作为研究对象,根据BMI的不同分为低体重组、正常体重组、超重组和肥胖组,各53例。应用生存分析中Kaplan-Meimer法比较各组的住院时间(以入院为事件开端,出院为终点事件);COX回归分析影响住院时间的因素。结果:AECOPD最短住院时间为6.25 d、最长为20 d;中位住院时间:低体重组为12.5 d,正常体重组为11.3 d,超重组为11.0 d,肥胖组为13.2 d。四组住院时间比较,差异有统计学意义(P<0.001)。正常体重组、超重组住院时间短于肥胖组,差异有统计学意义(P<0.001)。COPD稳定期Ⅰ级及Ⅱ级住院时间短于COPD稳定期Ⅲ级,差异有统计学意义(P<0.001)。结论:低体重、肥胖和COPD稳定期分级较高是增加AECOPD患者住院时间的因素。
Objective:To investigate the influence of body mass index(BMI)on the hospital stay length of patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods:From October 2007 to October 2022,212 patients with AECOPD admitted to the Department of Pulmonary Disease,Haian Municipal TCM Hospital were selected as the study subjects.They were divided into low weight group,normal weight group,overweight group and obesity group according to BMI,with 53 cases in each group.The Kaplan-Meimer method of survival analysis was used to compare the length of hospital stay in each group(admission as the start event and discharge as the end event);The factors affecting the length of hospital stay were analyzed using COX regression.Results:The shortest length of hospital stay for AECOPD was 6.25 d and the longest length was 20 d.Median length of hospital stay was 12.5 d in the low weight group,11.3 d in the normal weight group,11.0 d in the overweight group,and 13.2 d in the obesity group.There was a statistically significant difference in the length of hospital stay among four groups(P<0.001).The length of hospital stay in the normal weight group and overweight group was shorter than that in the obesity group,and the difference was statistically significant(P<0.001).The length of hospital stay of grade I and grade II COPD in the stable phase was shorter than that of grade II COPD in the stable phase,and the difference was statistically significant(P<0.001).Conclusion:Low weight,obesity and higher grade of COPD in the stable phase are the factors for prolonging the length of hospital stay of AECOPD patients.
