摘要
目的 探究麻黄、黄芩穴位贴敷肺俞穴对健康受试者颈胸部及背部穴位体表温度影响的差异,了解穴位贴敷疗法的作用机制。方法 招募10名健康受试者,每名受试者均进行3次贴敷,第一次贴敷麻黄粉0.5 g,为麻黄组;第二次贴敷黄芩粉0.5 g,为黄芩组;第三次贴敷麻黄+黄芩粉各0.5 g,为混合组。将准备好的药贴贴在受试者肺俞穴上,贴敷时间为3 h,每次贴敷间隔3天。贴敷前后应用红外热像仪对受试者双侧肺俞、双侧魄户、身柱、廉泉、天突和膻中进行热像图和体表温度信息采集。利用多元线性回归分析混杂因素(包括年龄、性别、体重、身高、体重指数以及贴敷后的体温、收缩压、舒张压、心率、血氧饱和度)对不同穴位的体表温度变化的影响。结果 红外热像图直观分析显示,黄芩组和混合组贴敷后受试者背部红外热像图红色面积扩大。红外热像图定量分析显示,贴敷前组间比较,左侧肺俞和左侧魄户穴位体表温度差异有统计学意义(P<0.05)。黄芩组贴敷后左侧肺俞、天突及膻中体表温度下降(P<0.05);混合组贴敷后双侧肺俞、双侧魄户、身柱、廉泉、天突和膻中体表温度均下降(P<0.05或P<0.01)。线性回归分析结果显示,黄芩组贴敷后天突体表温度下降0.46℃,差异有统计学意义(P<0.05)。年龄、体重指数、心率、舒张压对贴敷后穴位体表温度变化有一定影响(P<0.05或P<0.01)。结论 单独应用黄芩或麻黄联合黄芩贴敷于肺俞穴可能通过降低颈胸部及背部穴位体表温度的方式发挥穴位贴敷的治疗作用。
Objective To explore the differences in the effects of herbal patching of Mahuang(Herba Ephedrae)and Huangqin(Radix Scutellariae)on the surface temperature of neck,chest and back acupoints in healthy subjects,to study mechanism of acupoint herbal patching therapy.Methods Ten healthy subjects were recruited.Each subject was given herbal patching for three times;for the first time,O.5 g of Mahuang powder was used as the Mahuang group;for the second time,O0.5 g of Huangqin powder was used as the Huangqin group;for the third time,0.5 g of Mahuang powder and O.5 g of Huangqin powder were used as the mixed group.The herbal patch was applied at Feishu(BL13)acupoints,3 hours each time,with an interval of 3 days.Infrared thermal imager was used to collect thermal images and body surface temperature regarding bilateral Feishu(BL 13),bilateral Pohu(BL 42),Shenzhu(GV 12),Lianquan(CV 23),Tiantu(CV 22)and Danzhong(CV 17)before and after herbal patching.Multivariate linear regression analysis was performed to analyze the influence of confounding factors such as age,gender,weight,height,body mass index(BMI),body temperature,systolic blood pressure,diastolic blood pressure,heart rate,and blood oxygen saturation after herbal patching on the temperature changes at different acupoints.Results The visual analysis of infrared thermogram showed that the red area of the infrared thermogram on the back of the subject expanded in the Huangqin group and mixed group.Quantitative analysis of infrared thermogram showed that there was a statisti-cally significant difference in body surface temperature at the left Feishu(BL 13)and left Pohu(BL 42)before herbal patching among the groups(P<0.05).After herbal patching,the body surface temperature at left Feishu(BL 13),Tiantu(CV 22)and Danzhong(CV 17)in the Huangqin group decreased(P<0.05);the temperature at bilateral Feishu,bilateral Pohu,Shenzhu,Lianquan,Tiantu and Danzhong decreased in the mixed group(P<0.05 or P<0.O1).The linear regression analysis showed that the changes in temperature at Tian
