摘要目的评价氟康唑注射液耳浴联合曲安奈德益康唑乳膏对真菌性外耳道炎疗效及血清肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)及白细胞介素-6(IL-6)的影响。方法将2020年2月至2022年4月宜黄县人民医院收治的116例真菌性外耳道炎患者随机分为实验组(n=58)和参比组(n=58)。参比组采用曲安奈德益康唑乳膏治疗,实验组采用氟康唑注射液耳浴联合曲安奈德益康唑乳膏进行治疗,对比两组患者的治疗前后血清TNF-α、CRP及IL-6的炎症因子水平差异以及治疗有效性。结果治疗前两组CRP、IL-6、TNF-α差异无统计学意义(P<0.05);治疗后两组CRP、IL-6、TNF-α均较治疗前低(均P<0.05)且实验组的CRP、IL-6、TNF-α均低于参比组(均P<0.05);实验组总有效率96.55%高于参比组的86.21%(P<0.05)。结论使用氟康唑注射液耳浴联合曲安奈德益康唑乳膏治疗真菌性外耳道炎可有效改善患者血清TNF-α、CRP及IL-6炎症因子水平,减轻患者症状,提高治疗有效性,可在临床上借鉴应用。 Objective To evaluate the effect of fluconazole injection combined with triamcinolone acetonide and econazole nitrate cream on fungal external otitis media and serum tumor necrosis factor-α(TNF-α),C reactive protein(CRP)and interleukin-6(IL-6).Methods 116 patients with fungal external otitis media admitted to Yihuang County People's Hospital from February 2020 to April 2022 were randomly divided into experimental group(n=58)and reference group(n=58).The reference group was treated with triamcinolone acetonide and econazole nitrate cream,and the experimental group was treated with fluconazole injection ear bath combined with triamcinolone acetonide and econazole nitrate cream,and compared the differences in serum TNF-α,CRP and IL-6 and the treatment effectiveness.Results There were no statistically significant difference in TNF-α,CRP and IL-6 between the two groups before treatment(P<0.05);after treatment,the TNF-α,CRP and IL-6 levels in both groups were lower than before treatment(both P<0.05),and the experimental group were lower than those in the reference group(both P<0.05).The total effective rate of the experimental group was 96.55%,which was higher than 86.21%of the reference group.Conclusion The treatment of fluconazole injection ear bath combined with triamcinolone acetonide and econazole nitrate cream can effectively improve the serum levels of TNF-α,CRP and IL-6 inflammation,reduce the symptoms and improve the therapeutic effectiveness,which can be used in clinical practice.
