摘要
目的探究纤支镜肺泡灌洗联合糜蛋白酶治疗小儿重症肺炎的临床效果。方法方便选取2017年3月—2020年5月福建医科大学附属泉州第一医院收治的136例重症肺炎小儿随机均分为参照组和研究组,各68例。参照组采用纤支镜肺泡灌洗治疗,研究组采用纤支镜肺泡灌洗联合糜蛋白酶进行治疗,对比两组治疗效果和机体血气相应指标。结果治疗后研究组总有效率为97.06%,高于参照组的86.76%,差异有统计学意义(χ^(2)=4.847,P=0.028);治疗后,两组患儿PaO_(2)、血液pH与治疗前相比均升高,且研究组PO_(2)、血液pH均高于参照组,差异有统计学意义(P<0.05);治疗后,两组患儿Lac、PaCO_(2)较治疗前均降低,且研究组Lac、PaCO_(2)均低于参照组,差异有统计学意义(P<0.05)。结论使用纤支镜肺泡灌洗联合糜蛋白酶治疗应用于小儿重症肺炎,可有效改善患儿症状,提高治疗有效性,可在临床上借鉴应用。
Objective To explore the clinical effect of bronchoscopic alveolar lavage combined with chymotrypsin in the treatment of severe pneumonia in children.Methods One hundred and thirty-six eligible children with severe pneumonia admitted to Quanzhou First Hospital Affiliated to Fujian Medical University from March 2017 to May 2020 were conveniently selected and randomly divided into the reference group and the study group,68 cases in each group.The reference group was treated with bronchoscopic alveolar lavage,and the study group was treated with bronchoscopic alveolar lavage combined with chymotrypsin.The therapeutic effect and the corresponding indexes of blood gas were compared between the two groups.Results The total effective rate of the study group after treatment was 97.06%,higher than the control group's 86.76%,and the difference was statistically significant(χ^(2)=4.847,P=0.028).After treatment,PaO_(2) and blood pH values in both groups were increased compared with those before treatment,and PaO_(2) and blood pH values in the study group were higher than those in the reference group,and the difference was statistically significant(P<0.05).After treatment,Lac and PaCO_(2) in both groups were lower than before treatment,and Lac and PaCO_(2) in the study group were lower than those in the reference group,and the difference was statistically significant(P<0.05).Conclusion Fiberoptic alveolar lavage combined with chymotrypsin therapy can effectively improve the symptoms of children with severe pneumonia,improve the effectiveness of treatment,and can be used for reference in clinical practice.
