摘要
目的:分析多层螺旋CT(multi-slice spiral CT,MSCT)对肺孤立性磨玻璃结节(ground-glass nodule,GGN)良恶性的诊断价值。方法:选取2021年1月-2023年1月烟台市烟台山医院收治的927例GGN患者作为研究对象,所有患者均进行MSCT检查,以病理检查结果为诊断“金标准”,分析MSCT诊断GGN良恶性的价值。结果:病理结果证实,927例GGN患者中,305例(32.90%)为恶性,622例(67.10%)为良性。MSCT诊断结果显示恶性321例,良性606例,误诊37例,漏诊21例。以病理结果作为金标准,MSCT对GGN良恶性的诊断灵敏度为93.11%(284/305),特异度94.05%(585/622),准确率93.74%(869/927),阴性预测值96.53%(585/606),阳性预测值88.47%(284/321)。MSCT诊断结果与病理结果的一致性较好,Kappa=0.860。良恶性GGN的瘤肺界面、空泡征比较,差异无统计学意义(P>0.05),恶性GGN的形态不规则、含有实性成分、分叶征、毛刺征、胸膜凹陷征、血管集束征占比显著高于良性GGN(P<0.05)。结论:良恶性GGN的MSCT表现存在差异,MSCT对GGN良恶性的诊断价值较高。
Objective To analyze the value of multi-slice spiral CT(MSCT)in the differential diagnosis of benign and malignant ground-glass nodule(GGN).Methods A total of 927 GGN patients admitted to Yantai City Yantai Mountain Hospital from January 2021 to January 2023 were selected as research objects.All patients underwent MSCT examination,and pathological examination results were taken as the diagnostic"gold standard"to analyze the value of MSCT diagnosis in benign and malignant GGNs.Results Pathological results confirmed that among 927 GGN patients,305 cases(32.90%)were malignant and 622 cases(67.10%)were benign.The results of MSCT diagnosis showed that 321 cases were malignant,606 cases were benign,and misdiagnosed in 37 cases,and 21 cases were missed.The pathological results were taken as the gold standard,the sensitivity of MSCT was 93.11%(284/305),specificity 94.05%(585/622),accuracy 93.74%(869/927),negative predictive value 96.53%(585/606)and positive predictive value 88.47%(284/321)for the identification of benign and malignant GGNS.The consistency of MSCT diagnosis and pathological results was good,Kappa=0.860.There was no statistical significance in the comparison of tumor-lung interface and vacuolar sign between benign and malignant GGNs(P>0.05),but the proportion of irregular shape,solid component,lobular sign,burr sign,pleural sag sign and vascular cluster sign of malignant GGNs were significantly higher than that of benign GGNs(P<0.05).Conclusion There are differences in the MSCT manifestations of benign and malignant GGNs,and MSCT has high diagnostic value for benign and malignant GGNs.
