摘要
目的探讨肺磨玻璃密度结节患者的超高分辨率CT征象特点。方法从2022年1月至2023年4月在山东医学高等专科学校附属医院(临沂市老年病医院)收治的具有肺磨玻璃密度结节高分辨率CT征象患者中随机选择86例,根据其病理结果,分为良性组(32例)与恶性组(54例),并对其病案资料进行收集,进行回顾性分析。比较2组超高分辨率CT征象特点差异,包括病灶密度(单纯型、混合型);生长部位;病灶大小、形态;边缘(分叶征、毛刺征);结节与周围肺组织的界面(模糊、清楚光整、清楚毛糙);内部结构(空泡征、支气管征、囊状透亮影);临近结构改变(血管集束征、胸膜凹陷征)。结果良性组与恶性组的病灶密度、病灶形态、分叶征、毛刺征、结节与周围肺组织的界面、囊状透亮影等6种高分辨率CT征象特点的比较差异具有统计学意义(均P<0.05)。结论通过对超高分辨率CT征象特点的分析,对诊断肺磨玻璃密度结节的良、恶性有指导意义。
Objective To investigate the characteristics of ultra-high resolution CT findings in patients with pulmonary ground glass density nodules.Methods From January 2022 to April 2023,Eighty-six patients with high-resolution CT signs of pulmonary ground glass density nodules were randomly selected from the Affiliated Hospital of Shandong Medical College(Linyi Geriatric Hospital).According to the pathological results,they were divided into benign group(32 cases)and malignant group(54 cases),and their medical records were collected and retrospectively analyzed.The differences of ultra-high resolution CT features between the two groups were compared,including lesion density(simplex and mixed),growth site,size and shape of lesions,edge(lobulated sign,prickle sign),the interface between the nodule and the surrounding lung tissue(fuzzy,clear and smooth,clear and rough),internal structure(vacuole sign,bronchial sign,cystic translucent shadow),adjacent structural changes(vascular convergence sign,pleural depression sign).Results The malignant group and the benign group had statistically significant differences in six high-resolution CT signs,including lesion density,lesion morphology,lobulation sign,spicule sign,interface between nodules and surrounding lung tissue,and cystic translucent shadow(all P<0.05).Conclusion The analysis of ultra-high resolution CT features has guiding significance for the diagnosis of benign and malignant pulmonary ground glass density nodules.
