摘要
目的:探讨双参数MRI在前列腺良恶性病变中的诊断效能。方法:选取德州市第七人民医院及山东大学齐鲁医院德州医院2019年12月-2022年12月期间收治的56例前列腺癌患者作为恶性组,另选取同期前列腺增生患者66例作为良性组,所有患者均进行多模态MRI检查并测定其ADC值,重点评价T_(2)WI、DWI及T_(2)WI+DWI序列联合应用与病理诊断结果的一致性,以及诊断的灵敏度、特异度和准确率。结果:恶性组前列腺组织ADC值显著低于良性组(P<0.01)。T_(2)WI+DWI序列与病理结果一致性程度最高;恶性组T_(2)WI+DWI诊断灵敏度94.60%、特异度93.40%、准确率94.30%均高于T_(2)WI、DWI单一序列检查。结论:T_(2)WI+DWI序列联合应用在前列腺良恶性病变诊断中的效能最高。
Objective To investigate the diagnostic efficacy of two-parameter MRI in benign and malignant prostatic lesions.Methods A total of 56 patients with prostate cancer admitted in the Seventh People's Hospital of Dezhou and the Dezhou Hospital of Qilu Hospital of Shandong University from December 2019 to December 2022 were selected as the malignant group,and 66 patients with benign prostatic hyperplasia during the same period were selected as the benign group.All patients underwent multimodal MRI examination and their ADC values were measured.The consistency of T_(2)WI,DWI and combination of T_(2)WI and DWI with pathological results,as well as the sensitivity,specificity and accuracy of the diagnosis were evaluated.Results The ADC value of prostate tissue in the malignant group was significantly lower than that in the benign group(P<0.01).T_(2)WI+DWI sequence showed the highest consistency with pathological results.The sensitivity,specificity and accuracy of T_(2)WI+DWI in malignant group were 94.60%,93.40%and 94.30%,respectively,which were higher than those of T_(2)WI or DWI single sequence.Conclusion The combination of T_(2)WI and DWI has the highest efficiency in the diagnosis of benign and malignant prostate lesions.
