摘要
目的探讨小剂量艾司氯胺酮对鼻窦手术患者术后焦虑及抑郁情绪的影响。方法随机选取2021年6月—2022年5月中山市人民医院收治的40例择期行鼻窦手术患者为研究对象,随机分为对照组和干预组,每组20例。两组患者在诱导前分别静脉注射生理盐水3 mL和艾司氯胺酮0.2 mg/kg(用生理盐水配制成3 mL)。评估两组术前1 d及术后1、2 d医院焦虑抑郁量表(HADS)评分,记录两组患者术后30 min、24 h、48 h疼痛视觉模拟(VAS)评分及术后30 min Ramsay镇静评分,观察不良反应发生情况。结果术前1 d,两组患者焦虑及抑郁亚量表评分比较,差异无统计学意义(P>0.05)。术后2 d,干预组焦虑亚量表评分明显低于对照组,差异有统计学意义(t=2.140,P=0.039)。术后1、2 d,干预组抑郁亚量表评分明显低于对照组,差异有统计学意义(P<0.001)。术后30 min,干预组VAS评分(1.20±0.4)分低于对照组,差异有统计学意义(t=5.918,P<0.001),两组术后不良反应发生率比较,差异无统计学意义(χ^(2)=0.000,P=1.000)。结论小剂量艾司氯胺酮可以减轻鼻窦手术患者术后焦虑和抑郁情绪,且不增加不良反应发生率。
Objective To investigate the effect of low dose esketamine on postoperative anxiety and depression in patients undergoing sinus surgery.Methods Forty patients who received elective sinus surgery in Zhongshan People's Hospital from June 2021 to May 2022 were randomly selected as the study objects,and were randomly divided into control group and intervention group,with 20 patients in each group.Both groups were given intravenous injection of 3 mL normal saline and 0.2 mg/kg esketamine(3 mL prepared with normal saline),respectively,before induction.Hospital Anxiety and Depression Scale(HADS)scores 1 d before surgery and 1 d and 2 d after surgery were evaluated in both groups.Visual analog pain(VAS)scores 30 min,24 h and 48 h after surgery and Ramsay sedation scores 30 min after surgery were recorded in both groups,and the occurrence of adverse reactions was observed.Results 1 d before surgery,there was no statistically significant difference in subscale scores of anxiety and depression between the two groups(P>0.05).2 d after surgery,the anxiety subscale score of the intervention group was significantly lower than that of the control group,and the difference was statistically significant(t=2.140,P=0.039).1 and 2 d after surgery,the subscale score of depression in the intervention group was significantly lower than that in the control group,the difference was statistically significant(P<0.001).30 min after operation,VAS score of the intervention group(1.20±0.4)points was lower than that of the control group,the difference was statistically significant(t=5.918,P<0.001),and there was no statistically significant difference in the incidence of postoperative adverse reactions between the two groups(χ^(2)=0.000,P=1.000).Conclusion Low dose esketamine can reduce anxiety and depression in patients undergoing sinus surgery without increasing the incidence of adverse reactions.
