摘要
目的探究对比胰岛素不同给药方式联合二甲双胍治疗妊娠期糖尿病患者的效果及对血糖控制效果的影响。方法筛选湖北省荆门市京山市妇幼保健院在2021年5月—2022年4月收治的64例妊娠期糖尿病患者,随机分为两组,每组32例,比较观察组(胰岛素泵持续皮下注射+二甲双胍)和对照组(胰岛素常规皮下注射+二甲双胍)的治疗效果。结果治疗后,与对照组相比,观察组整体血糖水平、血糖恢复时间和胰岛素用量明显减少;观察组的血浆胰岛素(FINS)和胰岛素抵抗指数(HOMA-IR)较对照组显著降低,胰岛素β细胞(HOMA-β)较对照组明显提升;观察组的不良妊娠结局(6.25%)明显少于对照组(25.00%),差异有统计学意义(P<0.05)。结论妊娠期糖尿病患者接受胰岛素泵持续皮下注射+二甲双胍治疗,可有效降低血糖水平,改善胰岛素功能,保障妊娠结局,安全可靠。
Objective To explore and compare the effects of different administration methods of insulin,combined with metformin in the treatment of gestational diabetes mellitus and the effect on blood glucose control.Methods 64 patients with gestational diabetes mellitus admitted to Jingshan Maternal and Child Health Hospital from May 2021 to April 2022 were randomly divided into two groups with 32 cases in each group.The therapeutic effects of the observation group(continuous subcutaneous injection of insulin pump+metformin)and the control group(routine subcutaneous injection of insulin+metformin)were compared.Results After treatment,compared with the control group,the overall blood glucose level,blood glucose recovery time and insulin dosage were significantly reduced in the observation group;insulin(FNS)and insulin resistance(HOMA-IR)in observation group were significantly lower than control group,while insulinβcells(HOMA-β)was significantly higher than control group;the adverse pregnancy outcome in the observation group(6.25%)was significantly less than that in the control group(25.00%),and the difference was statistically significant(P<0.05).Conclusion In patients with gestational diabetes mellitus,continuous subcutaneous injection of insulin pump plus metformin can effectively reduce blood glucose level,improve insulin function and guarantee pregnancy outcome,which is safe and reliable.
