摘要
目的测定斑秃患者血清细胞因子IFN-γ、IL-12水平,调查斑秃患者生活质量与心理状态及其影响因素。方法采用酶联免疫吸附法(ELISA)检测100例斑秃患者血清IFN-γ、IL-12的水平,并对患者进行问卷调查评估,内容采用皮肤病生活质量指数(DLQI)以及流调中心抑郁量表(CES-D)。结果100例斑秃患者,DLQI评分为(6.91±5.53)分,总体上对生活质量的影响为中度影响,CES-D评分为(7.52±9.68),有抑郁倾向者占16.00%。家族史、文化程度、性别、月收入、病程、拔发试验、婚姻、年龄、就诊次数、复发,差异均无统计学意义(P>0.05);不同脱发面积,DLQI评分差异有统计学意义(P<0.05)。CES-D评分在年龄、婚姻方面,差异有统计学意义(P<0.05)。DLQI与CES-D评分之间存在正相关(r=0.578,P<0.01)。IFN-γ与IL-12呈正相关(r=0.730,P=0.000)。结论斑秃不但影响患者的生活质量,而且使患者心理产生不同程度的抑郁,对患者进行健康教育和采取适当的心理干预,有利于斑秃的治疗和康复。
Objective To measure the serum levels of cytokines IFN-γand IL-12 in patients with alopecia areata and investigate the quality of life and psychology and their influencing factors.Methods The cytokine levels of IFN-γand IL-12 were detected by E1ISA technique in 100 patients with alopecia areata.Patients with alopecia areata were evaluated by questionnaire using Dermatology Life Quality Index(DLQI)and Center for Epidemiologic studies Depression Scale(CES-D).Results The DLQI score of 100 alopecia areata patients was(6.91±5.53)score,CES-D score was(7.52±9.68)score.Alopecia areata had a moderate effect on the quality of life in general,and 16.00%of the patients showed depressive tendency.There was no significant difference in DLQI and CES-D of patients with different family history,education level,sex,monthly income,course and pulling hair test,marriage,age,number of visits,recurrence(P>0.05).There was significant difference in DLQI of patients with different hair loss area(P<0.05),and there was significant difference in CES-D of patients with different age and marriage(P<0.05).There is a positive correlation between DLQI and CES-D scores(r=0.578,P<0.01).The cytokine levels of IFN-γwas positively correlated with IL-12(r=0.730,P=0.000).Conclusion Alopecia areata can affects the quality of life,and may cause depression.Therefore,health education and appropriate psychological intervention are beneficial to them.
