Clinical Significance in Detecting Serum Changes of Th1/Th2 Related Cytokines in OSAHS Patients with Allergic Rhinitis

摘要 【目的】探讨辅助性T细胞亚群(Th1/Th2)相关细胞因子在阻塞性睡眠呼吸暂停低通气综合征(OSAHS)合并变应性鼻炎患者血清中的表达水平及其意义。【方法】选取2019年10月至2021年1月本院收治的105例OSAHS患者(观察组),根据是否合并变应性鼻炎分为合并组和未合并组;另选取同期来院体检的94例健康志愿者(对照组)。比较各组血清干扰素-γ(IFN-γ)、白细胞介素-2(IL-2)、白细胞介素-4(IL-4)、白细胞介素-13(IL-13)水平,分析OSAHS患者合并变应性鼻炎的影响因素及血清IFN-γ、IL-2、IL-4、IL-13水平对OSAHS患者合并变应性鼻炎的预测价值。【结果】观察组血清IFN-γ、IL-2水平均低于对照组(P<0.05),血清IL-4、IL-13水平均高于对照组(P<0.05);合并组血清IFN-γ、IL-2水平均低于未合并组(P<0.05),血清IL-4、IL-13水平均高于未合并组(P<0.05)。合并组患者呼吸暂停/低通气分级、腺样体病理性肥大构成比与未合并组患者比较,差异均有统计学意义(P<0.05);IFN-γ、IL-2、IL-4、IL-13水平及腺样体病理性肥大均是OSAHS患者合并变应性鼻炎的影响因素(P<0.05);血清IFN-γ、IL-2、IL-4、IL-13预测OSAHS患者合并变应性鼻炎的曲线下面积(AUC)分别为0.846,0.768,0.778,0.853。【结论】血清IFN-γ、IL-2、IL-4、IL-13均可作为预测OSAHS合并变应性鼻炎患者的敏感指标。 【Objective】To explore the clinical significance of changes in serum helper T cell subsets(Th1/Th2)related cytokines in patients with obstructive sleep apnea hypopnea syndrome(OSAHS)complicated with allergic rhinitis.【Methods】A total of 105 OSAHS patients who admitted to the hospital from October 2019 to January 2021 were selected for the study,and 94 healthy volunteers who came to the hospital for physical examination during the same period were selected.The levels of serum interferon-γ(IFN-γ),interleukin-2(IL-2),interleukin-4(IL-4)and interleukin-13(IL-13)in OSAHS patients and healthy individuals were measured and compared.As the situation of OSAHS with allergic rhinitis was considered,the factors affecting OSAHS patients with allergic rhinitis was analyzed.Thus,the value of serum IFN-γ,IL-2,IL-4,and IL-13 levels in predicting OSAHS patients with allergic rhinitis was investigated.【Results】Serum levels of both IFN-γand IL-2 in OSAHS patients were lower than those in healthy people(P<0.05),and serum IL-4 and IL-13 levels were higher than those in healthy people(P<0.05).The levels of serum IFN-γand IL-2 in patients with allergic rhinitis were lower than those without allergic rhinitis group(P<0.05),while the levels of serum IL-4 and IL-13 were higher than those without allergic rhinitis group(P<0.05).Compared to OSAHS patients without allergic rhinitis,the composition ratio of the apnea/hypopnea classification and pathological adenoid hypertrophy in patients with allergic rhinitis were significantly different(P<0.05).The levels of IFN-γ,IL-2,IL-4,IL-13 and pathological adenoid hypertrophy were related factors affecting OSAHS patients with allergic rhinitis(P<0.05).The area under the curve(AUC)of serum IFN-γ,IL-2,IL-4 and IL-13 levels in predicting OSAHS patients with allergic rhinitis were 0.846,0.768,0.778,and 0.853,respectively.【Conclusion】Clinical detection of serum IFN-γ,IL-2,IL-4,and IL-13 levels can be used as sensitive indicators to predict OSAHS patients with allergic rhinitis.
