沙格列汀联合阿卡波糖对糖尿病的血糖控制及糖化血红蛋白水平的影响 被引量:2

摘要 目的研究沙格列汀联合阿卡波糖对糖尿病的血糖控制及糖化血红蛋白水平的影响。方法以2019年1月至2022年9月周口市第二人民医院收治的100例2型糖尿病(Type 2 Diabetes,T2DM)患者为研究对象,采用随机数字表法分为常规组、试验组,每组各50例。常规组采用阿卡波糖治疗,试验组采用沙格列汀联合阿卡波糖治疗。比较两组临床疗效,治疗前、治疗3个月后血糖[空腹血糖(FPG)、糖化血红蛋白(HbA1c)、餐后2 h血糖(2 hPG)]水平,血糖波动指标[全天有效血糖波动次数(NGE)、平均血糖波动幅度(MAGE)、血糖标准差(SDBG)、日间血糖平均绝对差(MODD)],胰岛功能指标[胰岛素抵抗指数(HOMA-IR)、胰岛β细胞功能指数(HOMA-β)],血管内皮功能指标[一氧化氮(NO)、内皮素-1(ET-1)、颈动脉内膜中层厚度(IMT)]。结果试验组临床总有效率96.00%(48/50)高于常规组80.00%(40/50)(P<0.05)。治疗3个月后试验组HbA1c、FPG、2 hPG水平低于常规组(P<0.05);试验组SDBG、MAGE、MODD、NGE低于常规组(P<0.05);试验组HOMA-IR低于常规组,HOMA-β高于常规组(P<0.05);试验组NO高于常规组,ET-1、IMT低于常规组(P<0.05)。结论沙格列汀辅助治疗T2DM患者效果显著,可有效降低血糖波动,调节血糖,改善胰岛素β细胞功能及血管内皮功能。 Objective To study the effect of saxagliptin combined with acarbose on blood glucose control and glycosylated hemoglobin level in diabetes.Methods 100 patients with type 2 diabetes mellitus(T2DM)admitted to Zhoukou Second People's Hospital from January 2019 to September 2022 were used as study subjects,and patients were divided into conventional group and experimental group using the number table method,with 50 cases in each group.The conventional group was treated with acarbose,the experimental group was treated with saxagliptin and acarbose.Compare the clinical efficacy,blood glucose levels[fasting plasma glucose(FPG),glycosylated hemoglobin(HbA1c),2-hour postprandial blood glucose(2 hPG)],blood glucose fluctuation indicators[effective blood glucose fluctuations in frequency(NGE)],mean amplitude of glycemic excursions(MAGE),standard deviation of blood glucose(SDBG),mean absolute difference in daytime blood glucose(MODD)],islet function indicators[homeostasis model assessment-insulin resistance(HOMA-IR),index ofβ-cell function in HOMA(HOMA-β)],index of vascular endothelial function[nitric oxide(NO),Endothelin 1(ET-1),carotid intima-media thickness(IMT)].Results The total clinical effective rate of the experimental group was 96.00%(48/50)higher than that of the conventional group 80.00%(40/50)(P<0.05).After 3 months of treatment,the levels of HbA1c,FPG and 2 hPG in the experimental group were lower than those in the conventional group(P<0.05);the SDBG,MAGE,MODD and NGE in the experimental group were lower than those in the conventional group(P<0.05);the HOMA-IR in the experimental group was lower than that in the conventional group,HOMA-βwas higher than that of conventional group(P<0.05);NO in the experimental group was higher than that in the conventional group,and ET-1 and IMT were lower than those in the conventional group(P<0.05).Conclusion Saxagliptin is an effective adjunctive therapy for T2DM patients,which can effectively reduce blood glucose fluctuation,regulate blood glucose and improve insulinβCell
