沙格列汀与利格列汀治疗老年2型糖尿病患者血糖波动及血脂影响的对比分析 被引量:3

摘要 目的分析对比沙格列汀与利格列汀分别联合二甲双胍治疗老年2型糖尿病的血糖波动和血脂的影响及不良反应发生率。方法选取该院2019年9月—2020年10月收治的T2DM老年患者80例进行回顾性分析,按用药的不同分为对照组(沙格列汀联用二甲双胍)和观察组(利格列汀联用二甲双胍),每组40例。比较两组治疗前后血糖指标(FPG、2hPG、HbA1c)、血糖波动情况(SDBG、 PPGE、LAGE)、血脂指标(TG、TC、HDL-C、LDL-C)及不良反应发生率。结果观察组FPG、TG、LDL-C的改善程度均优于对照组,差异有统计学意义(P<0.05);对照组2 hPG、HbA1c、PPGE和LAGE改善程度高于观察组,差异有统计学意义(P<0.05);两组在对SDBG、TC、HDL-C的影响对比,差异无统计学意义(P>0.05);观察组不良反应发生率低于对照组,差异有统计学意义(P<0.05)。结论利格列汀在改善老年T2DM患者血脂方面优于沙格列汀;沙格列汀在改善老年T2DM患者血糖以及血糖波动方面大体优于利格列汀;不良反应发生率利格列汀比沙格列汀低。临床上应根据实际情况选用。 Objective To analyze and compare the effects of saxagliptin and linagliptin combined with metformin in the treatment of elderly type 2 diabetes with blood glucose fluctuations and blood lipids and the incidence of adverse reactions.Methods A retrospective analysis of 80 elderly patients with T2DM admitted to the hospital from September 2019 to October 2020 was selected. According to the different medications, they were divided into a control group(saxagliptin combined with metformin) and an observation group(linagliptin combined with metformin), with 40 cases in each group. The blood glucose indexes(FPG, 2 hPG, HbA1c), blood glucose fluctuations(SDBG, PPGE, LAGE), blood lipid indexes(TG, TC,HDL-C, LDL-C) and the incidence of adverse reactions were compared between the two groups before and after treatment.Results The degree of improvement of FPG, TG, LDL-C in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05). The improvement degree of 2 hPG, HbA1c, PPGE and LAGE in the control group was higher than that in the observation group, and the difference was statistically significant(P<0.05). There was no statistically significant difference between the two groups in the effects of SDBG, TC, and HDL-C(P>0.05). The incidence of adverse reactions in the observation group was lower than that in the control group, and the difference was statistically significant(P<0.05). Conclusion Linagliptin is better than saxagliptin in improving blood lipids in elderly patients with T2 DM. Saxagliptin is generally better than linagliptin in improving blood glucose and blood glucose fluctuations in elderly T2 DM patients. The adverse reaction rate of linagliptin is lower than that of saxagliptin. Clinically, it should be selected according to the actual situation.
