摘要
过敏性紫癜性肾炎最初由来自体内外之风邪引动而致病,发病之初未能及时遏制病势,致使未尽之风邪伏藏于体内,久居肾络,使气机郁滞,血络不通,肾络受损,毒邪、瘀血等病理产物互相搏结,终致微型癥瘕形成。先藏体内之伏风,遇外邪则发,遇内邪则动,从而导致患者病情缠绵难愈。风邪为本病病机演变过程中的关键因素,贯穿疾病始末,既是致病的始动因素,亦是迁延期的病理产物。临证应抓住“伏风”与“正虚”这两处关键,在治疗时提出“通”“补”并行之法,主张一方面通络除痹,透邪外出,一方面补气养血,托邪达外,双管齐下,使驱邪而不伤正,扶正而不留邪。
Henoch-Schnlein purpura nephritis is initially caused by Wind Evil factor from inside and outside of the body.At the beginning of the onset,if the disease isn′t cured in time,the remained Wind Evil factor hide in the body and linger in the Kidney collateral,causing Qi stagnation and blood stasis,the collateral being blocked and the Kidney collateral being damaged.When the pathogens,blood stasis,and other pathological products tangled with each other,miniature lumps would be formed.The hidden latent Wind in the body bursts when it encounters external pathogens,and moves when it encounters internal pathogens,thus making the patient′s condition lingering and difficult to be healed.Therefore,Wind is the key Evil factor in the pathogenesis evolution,which runs through the whole process of the disease.It is not only the initiating factor of the disease but also the pathological product of the protracted period.In clinical practice,the two key points of"latent Wind"and"Zheng Qi deficiency"should be grasped and the"dredging"and"nourishing"method should be used simultaneously during treatment.It is advocated a two-pronged approach:on the one hand,dredging the collaterals and removing numbness to expelling the evil pathogens out.On the other hand,nourishing Qi and blood and dispelling the Evil factors out.As a result,the Evil factors can be expelled without harming the vital Qi,and the healthy Qi can be strengthened without retaining the evil pathogens.
